摘 要:目的:在口腔鳞状细胞癌不同区域间质细胞中,研究软骨寡聚基质蛋白(cartilage oligomeric matrix protein, COMP)的表达及临床意义。方法:首先运用TCGA数据库分析COMP基因表达情况。收集104例手术切除的口腔鳞癌组织,采用免疫组化技术,检测肿瘤中心和浸润前沿区域成纤维样细胞和淋巴细胞中COMP的表达。结果:TCGA数据库和免疫组化结果显示肿瘤组织中COMP表达水平显著高于癌旁组织。不同区域间质细胞的表达分析显示COMP在浸润前沿成纤维样细胞的表达水平显著高于肿瘤中心。COMP在肿瘤中心浸润淋巴细胞和浸润前沿成纤维样细胞的表达均与患者复发显著相关,在不同区域肿瘤浸润淋巴细胞的表达均与神经侵犯相关。Kaplan-Meier生存分析显示COMP在浸润前沿成纤维样细胞和肿瘤中心浸润淋巴细胞高表达与患者生存率降低显著相关。Cox回归分析显示COMP在浸润前沿成纤维样细胞高表达与患者无病生存率降低显著相关。结论:口腔鳞癌组织浸润前沿成纤维样细胞COMP的高表达可能是影响患者预后的独立危险因素。Objective:Expression and clinical significance of cartilage oligomeric matrix protein(COMP)was studied in mesenchymal cells from different regions of oral squamous cell carcinoma(OSCC).Methods:COMP gene expression was first analyzed using the The Cancer Genome Atlas(TCGA)database.Paraffin-embedded sections of 104 OSCC tissues were collected at Nanjing Stomatological Hospital.The sections were stained by immunohistochemistry(IHC)and focused on fibroblast-like cells(FLCs)and tumor-infiltrating lymphocytes(TILs)at tumor center(TC)and invasive tumor front(ITF)area.Results:TCGA dataset and IHC results showed that the expression level of COMP was significantly higher in OSCC tissues than that in adjacent tissues.Moreover, regional expression analysis suggested that COMP expression in FLCs were higher at ITF than at TC.Chi-square tests revealed that COMP expression in TILs at TC and in FLCs at ITF were associated with relapse, and COMP expression in TILs both at TC and ITF were associated with depth of invasion(DOI).Kaplan-Meier survival analysis indicated that high expression of COMP in FLCs at IFT and in TILs at TC significantly associated with poor survivals of OSCC patients.Cox regression analysis illustrated that high expression of COMP in FLCs at IFT significantly associated with reduced disease-free survival(DFS).Conclusion:High expression of COMP in FLCs at invasive tumor front area could be an independent risk factor affecting prognosis of OSCC patients.
