术前血清CA125和腹水对胰腺癌腹膜转移诊断价值

摘　　要：目的 探讨术前血清肿瘤标志物CA125和腹水与胰腺癌腹膜转移之间的关联性,为胰腺癌腹膜转移的早期诊断提供依据。方法 选取2018年1月—2021年12月在我院因胰腺癌腹膜转移行外科手术的病人24例,无腹膜转移的胰腺癌病人89例。收集病人的CA125及腹水状态等指标,分析其与腹膜转移的关系。结果 有无腹膜转移的胰腺癌病人的分化程度、CA199、CA125及腹水状态的差别有统计学意义(χ^(2)=6.43~9.54,P<0.05)。CA125(OR=1.189,95%CI=1.040~1.358)及腹水(OR=1.037,95%CI=0.806~1.225)为胰腺癌腹膜转移的独立危险因素。CA125+腹水诊断胰腺癌腹膜转移的受试者工作特征曲线下面积(0.953)大于CA125(0.941)和腹水(0.859)单独诊断(Z=3.950、4.158,P<0.001)。结论 CA125及腹水是胰腺癌腹膜转移的独立危险因素,二者联合检测可提高胰腺癌腹膜转移的诊断准确性。Objective To investigate the association of the preoperative serum tumor marker CA125 and ascites with the peritoneal metastasis of pancreatic cancer,and to provide a basis for the early diagnosis of peritoneal metastasis of pancreatic can-cer.Methods A total of 24 patients who underwent surgical operation in our hospital due to peritoneal metastasis of pancreatic cancer from January 2018 to December 2021 were enrolled,as well as 89 pancreatic cancer patients without peritoneal metastasis.The indicators such as CA125 and ascites status were collected to analyze their association with peritoneal metastasis.Results There were significant differences in the degree of tumor differentiation,CA199,CA125,and ascites status between the pancreatic cancer patients with peritoneal metastasis and those without peritoneal metastasis(χ^(2)=6.43-9.54,P<0.05).CA125(OR=1.189,95%CI=1.040-1.358)and ascites(OR=1.037,95%CI=0.806-1.225)were independent risk factors for peritoneal metastasis of pancreatic cancer.The combination of CA125 and ascites had a significantly larger area under the ROC curve than CA125 or ascites alone(0.953 vs 0.941/0.859,Z=3.950,4.158,P<0.001).Conclusion CA125 and ascites are independent risk factors for peritoneal metastasis of pancreatic cancer,and combined measurement of CA125 and ascites can improve the diagnostic accuracy of peritoneal metastasis of pancreatic cancer.
