摘  要:目的观察纤溶酶联合丹红注射液治疗对急性缺血性脑血管病(AICVD)患者脑血流参数及血清缺氧诱导因子-1α(HIF-1α)、和肽素(copeptin)水平的影响。方法选取2021年8月至2023年8月海安市中医院药剂科AICVD患者98例,采用随机数字表法分为联合组(n=49)、纤溶酶组(n=49)。纤溶酶组给予纤溶酶治疗,联合组在纤溶酶组基础上联用丹红注射液,均治疗2 w。比较两组治疗前后峰流速(Vp)、平均流速(Vm)、搏动指数(PI)、脑血管储备能力(CAR)、血清低氧诱导因子-1α(HIF-1α)、血管生成素-2(Ang-2)、血管内皮生长因子(VEGF)、和肽素(copeptin)、中枢神经特异蛋白(S-100β)、脑源性神经营养因子(BDNF)、神经元特异性烯醇化酶(NSE)水平、美国国立卫生研究院卒中量表(NHISS)、Barthel指数(BI)评分,并统计治疗结束后两组的疗效。结果两组治疗后PI、血清HIF-1α、copeptin、S-100β、NSE水平及NIHSS评分均降低,Vp、Vm、CAR、血清Ang-2、VEGF、BDNF水平及BI评分均增高,且联合组治疗后PI、血清HIF-1α、VEGF、copeptin、S-100β、NSE水平及NIHSS评分下降幅度更大,Vp、Vm、CAR、血清Ang-2、BDNF水平及BI评分增高幅度更大(P<0.05);治疗结束后,联合组总有效率93.88%高于纤溶酶组79.59%(P<0.05)。结论纤溶酶联合丹红注射液治疗可改善AICVD患者脑血流灌注及脑组织低氧缺血状态,促进神经功能及日常生活能力恢复,疗效确切。Objective To observe the effect of fibrinolytic enzyme combined with Danhong injection on cerebral blood flow parameters and serum hypoxia-inducible factor-1α(HIF-1α)and copeptin levels in patients with acute ischemic cerebrovascular disease(AICVD).Methods A total of 98 patients with AICVD in Department of Pharmacy,Hai'an Hospital of Traditional Chinese Medicine from August 2021 to August 2023 were selected and divided into combined group(n=49)and fibrinolytic enzyme group(n=49)by random number table method.The fibrinolytic enzyme group was treated with fibrinolytic enzyme,and the combined group was treated with Danhong injection on the basis of the fibrinolytic enzyme group.Both groups were treated for 2 weeks.The peak velocity(Vp),mean velocity(Vm),pulsatility index(PI),cerebrovascular reserve(CAR),serum hypoxia-inducible factor-1α(HIF-1α),angiopoietin-2(Ang-2),vascular endothelial growth factor(VEGF),copeptin,central nervous system specific proteinsβ(S-100β),brain-derived neurotrophic factor(BDNF),neuron-specific enolase(NSE)levels,National Institutes of Health Stroke Scale(NHISS)and Barthel index(BI)scores were compared between the two groups before and after treatment.And the efficacy of the two groups after the end of treatment was counted.Results After treatment,the levels of PI,serum HIF-1α,copeptin,S-100β,NSE and NIHSS scores in the two groups decreased,while the levels of Vp,Vm,CAR,serum Ang-2,VEGF,BDNF and BI scores increased.After treatment,the levels of PI,serum HIF-1α,VEGF,copeptin,S-100β,NSE and NIHSS scores in the combined group decreased more significantly,while the levels of Vp,Vm,CAR,serum Ang-2,BDNF and BI scores increased more significantly(P<0.05).After treatment,the total effective rate of the combined group was 93.88% higher than that of the fibrinolytic enzyme group 79.59%(P<0.05).Conclusion Fibrinolytic enzyme combined with Danhong injection can improve cerebral blood perfusion and cerebral hypoxia-ischemia in patients with AICVD,and promote the recovery of neurological functio
