涤痰汤合桃红四物汤加减治疗痰瘀阻络型脑梗死恢复早期临床观察

摘　　要：目的:观察涤痰汤合桃红四物汤加减治疗痰瘀阻络型脑梗死恢复早期的临床疗效。方法:选取符合纳入标准的痰瘀阻络型脑梗死恢复早期患者96例,采用随机数字表法分为两组各48例。对照组予常规对症治疗及个体化康复综合训练,治疗组在对照组基础上予涤痰汤合桃红四物汤加减治疗。比较两组神经功能缺损程度、中医证候积分、神经损伤因子水平、脑血流动力学指标水平。结果:治疗后,两组卒中量表(NIHSS)评分、中医证候积分、各项神经损伤因子水平、各项脑血流动力学指标水平均较治疗前改善(P﹤0.05),且治疗组改善幅度均优于对照组(P﹤0.05)。结论:涤痰汤合桃红四物汤加减对痰瘀阻络型脑梗死恢复早期脑神经的保护作用较好,可有效改善脑血流动力学。Objective:To observe the clinical efficacy of Ditan decoction and modified Taohong Siwu decoction of early convalescent cerebral infarction with syndrome of phlegm and blood stasis obstructing collaterals.Methods:96 cases of eligible patients with early convalescent cerebral infarction with syndrome of phlegm and blood stasis obstructing collaterals were selected and randomly divided into two groups,48 cases in each group.The control group was treated by conventional symptomatic therapy and individualized comprehensive rehabilitation training,while the treatment group was treated plus with Ditan decoction and modified Taohong Siwu decoction on the basis of control group.The degree of neurologic impairment,TCM syndrome score,level of neurologic impaired factors and level of cerebral hemodynamics indexes were compared between two groups.Results:After treatment in both groups,the scores of National Institute of Health Stroke Scale(NIHSS),TCM syndrome scores,levels of various neurologic impaired factors and levels of various cerebral hemodynamics indexes were improved compared with before treatment(P﹤0.05),and the improvement of treatment group was better than that of control group(P﹤0.05).Conclusion:The protective effect of Ditan decoction and modified Taohong Siwu decoction on cerebral nerves of early convalescent cerebral infarction with syndrome of phlegm and blood stasis obstructing collaterals is more significant,which can effectively improve patients'cerebral hemodynamics.
