摘 要:目的:探究早期低级别子宫内膜样癌中错配修复蛋白表达缺失的临床病理意义,以期为早期低级别子宫内膜样癌的诊断与治疗措施的制订提供参考。方法:选取2021年2月—2024年2月赣州市妇幼保健院收治的40例早期低级别子宫内膜样癌患者为观察组,同时期收治的40例高级别子宫内膜样癌患者为对照组。采用免疫组织化学染色法检测所有病变组织中4种错配修复蛋白[减数分裂后分离增强蛋白2(PMS2)、MutL同源物1(MLH1)、MutS同源物2(MSH2)及MutS同源物6(MSH6)]的表达情况,并比较两组及观察组中不同年龄、体重指数(BMI)、国际妇产科联盟(FIGO)分期及组织学分级患者错配修复蛋白的表达缺失情况。结果:观察组PMS2、MLH1及MSH2表达缺失率均显著低于对照组,差异均有统计学意义(P<0.05);两组MSH6表达缺失率比较,差异无统计学意义(P>0.05);观察组中不同年龄、FIGO分期及组织学分级患者PMS2、MLH1、MSH2及MSH6表达缺失率比较,差异均有统计学意义(P<0.05);观察组中不同BMI患者PMS2、MLH1、MSH2及MSH6表达缺失率比较,差异均无统计学意义(P>0.05)。结论:早期低级别子宫内膜样癌中错配修复蛋白表达缺失率均显著低于高级别子宫内膜样癌,且不同年龄、FIGO分期及组织学分级低级别子宫内膜样癌患者的错配修复蛋白表达缺失情况存在显著差异。Objective:To explore the clinical pathological significance of expression deficiency of mismatch repair proteins in early low-grade endometrioid carcinoma,in order to provide reference for the diagnosis and formulation of treatment measures of early low-grade endometrioid carcinoma.Method:A total of 40 patients with early low-grade endometrioid carcinoma admitted to Ganzhou Women and Children's Health Care Hospital from February 2021 to February 2024 were selected as the observation group,and 40 patients with high-grade endometrioid carcinoma admitted during the same period were selected as the control group.Immunohistochemical stain was used to detect the expression of four mismatch repair proteins[post meiotic segregation 2(PMS2),MutL homologue 1(MLH1),MutS homologue 2(MSH2)and MutS homologue 6(MSH6)]in all the pathological tissues.The expression deficiency of mismatch repair proteins in the two groups and in patients with different ages,body mass index(BMI),federation international of gynecology and obstetric(FIGO)stages and histological grades in the observation group were compared.Result:The expression deficiency rates of PMS2,MLH1 and MSH2 in observation group were significantly lower than those in control group,the differences were statistically significant(P<0.05).There was no significant difference in the expression deficiency rate of MSH6 between the two groups(P>0.05).There were significant differences in the expression deficiency rates of PMS2,MLH1,MSH2 and MSH6 among patients with different ages,FIGO stages and histological grades(P<0.05).There were no significant differences in the expression deficiency rates of PMS2,MLH1,MSH2 and MSH6 among patients with different BMI in the observation group(P>0.05).Conclusion:The expression deficiency rates of mismatch repair proteins in early low-grade endometrioid carcinoma are significantly lower than those in high-grade endometrioid carcinoma,and there are significant differences in the expression deficiency rates of mismatch repair proteins in low-grade endo
