达芬奇机器人辅助早期卵巢癌全面分期术术后并发症影响因素分析

摘　　要：目的:探讨达芬奇机器人辅助系统在早期卵巢癌全面分期手术并发症的发生情况及影响因素。方法:回顾分析2014年10月至2023年2月郑州大学第一附属医院收治的达芬奇机器人辅助早期卵巢癌全面分期手术患者的临床资料,统计术后并发症发生情况,采用单因素和多因素logistic回归分析。结果:完成机器人辅助早期卵巢癌全面分期术149例,39例(26.2%)发生术后并发症,包括深静脉血栓形成12例(8.1%)、淋巴潴留囊肿7例(4.7%)、盆腔脓肿4例(2.7%)、腹壁切口疝4例(2.7%)、肠梗阻4例(2.7%)、肺栓塞3例(2.0%)、肾积水3例(2.0%)、腹腔内出血2例(1.3%)。单因素分析结果显示,达芬奇机器人辅助早期卵巢癌全面分期术后总体并发症发生与年龄、BMI、手术时间及合并疾病(高血压、冠心病、糖尿病)有关(P<0.05)。多因素分析结果显示,年龄、BMI及手术时间是术后总体并发症发生的独立影响因素(P<0.05),BMI、手术时间及合并疾病(高血压、冠心病、糖尿病)是术后近期并发症发生的独立影响因素(P<0.05)。结论:年龄、BMI、手术时间是达芬奇机器人辅助早期卵巢癌全面分期术术后总体并发症发生的独立影响因素。Objective:To study the incidence and influence factors of complications in Da Vinci robotic-assisted laparoscopy in the comprehensive staging surgery of early ovarian cancer.Methods:The clinical data of patients with early ovarian cancer who underwent Da Vinci robotic-assisted comprehensive staging surgery from October 2014 to February 2023 in the First Affiliated Hospital of Zhengzhou University were retrospectively analyzed.The incidence of postoperative complications was analyzed with univariate analysis and multivariate logistic regression analysis.Results:149 cases were included in this study,and 39 cases(26.2%)suffered postoperative complications,including 12(8.1%)deep vein thrombosis,7(4.7%)lymphocele,4(2.7%)pelvic abscess,4(2.7%)abdominal incisional hernia,4(2.7%)intestinal obstruction,3(2.0%)pulmonary embolism,3(2.0%)hydronephrosis and 2(1.3%)intraperitoneal hemorrhage.Univariate analysis showed that age,body mass index(BMI),operating time and comorbidities(hypertension,coronary heart disease,diabetes mellitus)were related to the incidence of complications(P<0.05).Multivariate analysis showed that age,BMI and operating time were independent risk factors of complications(P<0.05).Multivariate analysis showed that BMI,operating time and comorbidities(hypertension,coronary heart disease,diabetes mellitus)were the independent risk factors of short-term postoperative complications.Conclusion:Age,BMI and operating time are influencing factors to complications happens after comprehensive staging surgery of early ovarian cancer.
