莫沙必利联合雷贝拉唑和磷酸铝凝胶治疗老年反流性食管炎的效果

摘　　要：目的研究莫沙必利联合雷贝拉唑和磷酸铝凝胶治疗老年反流性食管炎(RE)的效果。方法队列研究。抽取安阳市人民医院2020年1月至2023年12月收治的老年RE患者70例,按治疗方法分为对照组和研究组,每组35例。对照组采用莫沙必利,研究组在对照组的治疗基础上联合雷贝拉唑和磷酸铝凝胶。比较两组疗效、症状消退时间、治疗前后胃分泌指标[胃泌素(G-17)、胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶原Ⅱ(PGⅡ)]水平及不良反应发生率。结果研究组总有效率(94.29%,33/35)高于对照组(68.57%,24/35),P<0.05。研究组胃部烧灼感、反流、上腹部疼痛消退时间短于对照组(P<0.05)。治疗前,两组G-17、PG I、PG II水平比较差异未见统计学意义(P>0.05);治疗后,研究组G-17、PG I水平高于对照组,差异有统计学意义(P<0.05)。研究组不良反应发生率(11.43%,4/35)与对照组(5.71%,2/35)比较差异未见统计学意义(P>0.05)。结论莫沙必利联合雷贝拉唑和磷酸铝凝胶治疗老年RE的效果显著,能减轻机体氧化应激反应,快速改善胃部烧灼感、反流、上腹部疼痛等症状,恢复患者胃分泌功能,且治疗安全性良好。Objective To investigate the effect of mosapride combined with rabeprazole and aluminium phosphate gel in the treatment of elderly patients with reflux esophagitis(RE).Methods A total of 70 elderly patients with RE treated in Anyang People’s Hospital from January 2020 to December 2023 were selected for the cohort study,and they were divided into a control group and a study group according to treatment method,with 35 cases in each group.The control group was treated with mosapride,and the study group was treated with rabeprazole combined with aluminium phosphate gel based on the treatment of the control group.The therapeutic effect,symptom resolution time,preoperative and postoperative levels of gastric secretion indicators,including gastrin 17(G-17),pepsinogen I(PG I)and pepsinogen II(PG II),and the incidence of adverse reactions were compared between the two groups.Results The total effective rate of the study group(94.29%,33/35)was higher than that of the control group(68.57%,24/35),P<0.05.The resolution time of epigastric burning sensation,reflux and upper abdominal pain in the study group was shorter than that in the control group(P<0.05).There was no significant difference in the levels of G-17,PG I and PG II between the two groups before treatment(P>0.05).After treatment,the study group had higher levels of G-17 and PG I compared with the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the study group(11.43%,4/35)and the control group(5.71%,2/35),P>0.05.Conclusions Mosapride combined with rabeprazole and aluminium phosphate gel is effective in the treatment of elderly RE.This protocol can reduce oxidative stress response,quickly alleviate symptoms(epigastric burning sensation,reflux,upper abdominal pain),and restore gastric secretion function,with good treatment safety.
