摘　　要：骨髓间充质干细胞(BMSCs)作为一种新型脑梗死的治疗手段,近年来备受关注。BMSCs主要通过替代及填充、免疫调节、抑制凋亡以及保护血脑屏障等多种途径发挥治疗作用。本文系统地对BMSCs治疗脑梗死的作用机制、基因修饰的BMSCs和BMSCs来源的外泌体治疗脑梗死的作用机制、BMSCs联合其他方法治疗脑梗死的作用机制进行了综述,并对BMSCs移植的途径、时间及剂量,目前BMSCs治疗脑梗死的临床应用情况进行了总结,为今后进一步临床研究提供参考。Bone marrow mesenchymal stem cells(BMSCs)have attracted much attention in recent years as a novel therapeutic tool for cerebral infarction.BMSCs exert therapeutic effects primarily through multiple pathways,including replacement and filling,immunomodulation,inhibition of apoptosis,and protection of the blood-brain barrier.This article systematically reviews the mechanisms of BMSCs in the treatment of cerebral infarction,including the mechanisms of genemodified BMSCs and BMSCs-derived exosome therapy,as well as the mechanisms of BMSCs in combination with other methods of treatment for cerebral infarction.Additionally,the article summarizes the routes,timing and dosages of BMSC transplantation,and the current clinical application of BMSCs in the treatment of cerebral infarction to provide a reference for future clinical research.
