胆囊癌组织LncRNA ITGβ1表达与临床病理特征、切除术后复发的关系研究

摘　　要：目的:检测胆囊癌组织长链非编码核糖核酸整合素β1(LncRNA ITGβ1)表达,并分析其临床意义。方法:选取行手术治疗的胆囊癌患者124例,选取切除的癌旁组织作为对照。检测组织LncRNA ITGβ1表达;比较癌组织、癌旁组织LncRNA ITGβ1表达;比较不同临床病理特征患者癌组织LncRNA ITGβ1表达。随访至2023年9月,统计患者复发情况。比较无复发组、复发组一般资料及癌组织LncRNA ITGβ1表达;用COX回归分析胆囊癌切除术后复发的影响因素;绘制Kaplan-Meier曲线分析癌组织LncRNA ITGβ1表达与胆囊癌切除术后复发的关系。结果:癌组织LncRNA ITGβ1相对表达量高于癌旁组织(P<0.05);肿瘤、淋巴结和转移(TNM)Ⅲ/Ⅳ期、未/低分化、浸润深度T_(3)/T_(4)患者癌组织LncRNA ITGβ1表达高于TNM分期Ⅰ/Ⅱ期、中/高分化、浸润深度T_(1)/T_(2)患者(P<0.05);109例患者随访12~36个月,中位随访时间23个月,复发率为48.62%;Cox回归分析显示,肿瘤位置颈部或胆囊管(HR=2.433,95%CI:1.307~4.528)、TNM分期Ⅲ/Ⅳ期(HR=2.625,95%CI:1.580~4.361)、未/低分化(HR=2.782,95%CI:1.671~4.630)、术后治疗不依从(HR=2.186,95%CI:1.195~3.998)、癌组织LncRNA ITGβ1表达(HR=2.098,95%CI:1.437~3.063)是胆囊癌切除术后复发的影响因素(P<0.05);Log-rank检验显示,LncRNA ITGβ1高表达患者的复发率高于LncRNA ITGβ1低表达患者(P<0.05)。结论:胆囊癌组织LncRNA ITGβ1表达高于癌旁组织,其与TNM分期、分化程度及浸润深度有关,且是胆囊癌切除术后复发的影响因素。Objective:To detect the expression of long non-coding RNA integrinβ1(LncRNA ITGβ1)in gallbladder cancer tissues and analyze its clinical significance.Methods:124 patients with gallbladder cancer who underwent surgical treatment were selected,and the adjacent tissues removed were used as controls.The expression of LncRNA ITGβ1 in tissues was detected.The expression of LncRNA ITGβ1 in cancer tissues and adjacent tissues was compared.The expression of LncRNA ITGβ1 in cancer tissues of patients with different clinical pathological characteristics was compared.Follow-up until September 2023,and the recurrence of patients was recorded.The general data and LncRNA ITGβ1 expression in cancer tissues of the non-recurrence group and the recurrence group were compared.COX regression analysis was used to analyze the influencing factors of recurrence after gallbladder cancer resection.Kaplan-Meier curve was drawn to analyze the relationship between LncRNA ITGβ1 expression in cancer tissues and recurrence after gallbladder cancer resection.Results:The relative expression of LncRNA ITGβ1 in cancer tissues was higher than that in adjacent tissues(P<0.05).The expression of LncRNA ITGβ1 in cancer tissues in TNM stageⅢ/Ⅳ,undifferentiated/low differentiation,infiltration depth T_(3)/T_(4) patients were higher than those in TNM stageⅠ/Ⅱ,moderate/high differentiation,infiltration depth T_(1)/T_(2) patients(P<0.05).109 patients were followed up for 12 to 36 months,with a median follow-up time of 23 months,the recurrence rate was 48.62%.Cox regression analysis showed that cervical or gallbladder duct(HR=2.433,95%CI:1.307~4.528),TNM stageⅢ/Ⅳ(HR=2.625,95%CI:1.580~4.361),no/low differentiation(HR=2.782,95%CI:1.671~4.630),non-adherence to postoperative treatment(HR=2.186,95%CI:1.195~3.998),LncRNA ITGβ1 expression in cancer tissues(HR=2.098,95%CI:1.437~3.063)were influential factors for postoperative recurrence of gallbladder cancer(P<0.05).Log-rank test showed that the recurrence rate of patients with high expression of
