摘  要:蕈样肉芽肿病(MF,又称蕈样霉菌病)/Sezary综合征是皮肤T细胞淋巴瘤的一种。全身皮肤照射已经被证明是一种治疗皮肤MF的有效方式。既往一般使用电子线治疗,全身皮肤电子治疗剂量与处方剂量的偏差高达40%。随着放疗技术的进步,光子螺旋断层放射治疗在技术上可以代替电子线治疗,其优点包括:明显改善剂量均匀性、不存在射野衔接、患者在更加舒适的仰卧位进行治疗。本文根据中山大学肿瘤防治中心的治疗数据,联合多家放射治疗中心,就光子螺旋断层放射治疗MF,从物理、技术、临床三个方面,对体位固定、CT模拟定位、放疗计划设计和剂量处方等放射治疗的各个环节,提供尽量统一的规范和标准,共各医疗单位参考,使MF患者获得最为合理的放射治疗。Mycosis fungoides(MF)/Sezary syndrome is a type of cutaneous T-cell lymphoma.Whole-body skin irradiation has been proven to be an effective treatment for cutaneous MF.In the past,electron beam therapy was generally used,and the deviation of whole-body skin electrotherapy from the prescribed dose was as high as 40%.With the advancement of radiotherapy technology,photon spiral tomography can technically replace electron beam therapy,and its advantages are as follows.It significantly improves dose uniformity.There is no shooting field connection.Patients can be treated in a more comfortable supine position.Based on the treatment data from Sun Yat-sen University Cancer Center,cooperating with a number of radiotherapy centers,the application of photon spiral tomography radiotherapy in the treatment of MF was analyzed,aiming to unify all aspects of radiotherapy such as body position fixation,CT simulation positioning,radiotherapy plan design and dose prescription from the physical,technical and clinical aspects,thereby providing reference for medical institutions and enabling MF patients to receive the optimal radiotherapy.
