摘 要:目的分析2018-2022年泰安市居民恶性肿瘤死亡概况及潜在减寿年数(PYLL),为政府制定恶性肿瘤防治措施提供数据支持。方法数据来源于2018-01-01-2022-12-31医疗机构上报的恶性肿瘤死亡个案,用粗死亡率、标化死亡率、构成比、年度变化百分比(APC)、PYLL、平均减寿年数(AYLL)等指标进行统计分析。结果2018-2022年泰安市恶性肿瘤粗死亡率为204.48/10万,标化死亡率为104.69/10万,由恶性肿瘤导致的死亡占全死因的26.49%。男性恶性肿瘤粗死亡率为272.06/10万,标化死亡率为150.26/10万,女性恶性肿瘤粗死亡率为136.48/10万,标化死亡率为64.63/10万,男性死亡率高于女性,差异有统计学意义,χ^(2)=6274.968,P<0.001;全人群、男性、女性恶性肿瘤标化死亡率呈现下降趋势,但差异无统计学意义,APC分别为-2.105(t=-2.931,P=0.061)、-1.757(t=-2.651,P=0.077)和-2.319(t=-2.328,P=0.102)。从年龄趋势看,30岁之后恶性肿瘤逐渐上升,尤其是50岁之后,粗死亡率快速上升,最后在≥85岁年龄组达到高峰。男性和女性恶性肿瘤前5位死亡顺位与全人群一致,依次是肺癌、食管癌、胃癌、肝癌和结直肠癌。男性和全人群恶性肿瘤PYLL前5位顺位相同,依次是肺癌、肝癌、食管癌、胃癌、结直肠癌,前5位中AYLL最高的是肝癌,其次是结直肠癌,女性恶性肿瘤PYLL前5位依次是肺癌、乳腺癌、肝癌、胃癌和食管癌,AYLL最高的是乳腺癌,其次是肝癌。结论泰安市恶性肿瘤死亡率较高,疾病负担较重,癌症防治任务艰巨,应重视癌症早诊早治,以期降低疾病负担。Objective To analyze death and potential years of life lost(PYLL)of malignant tumor from 2018 to 2022 in Taian city,and provide data support for the government to formulate prevention and control measures for malignant tumors.Methods The data sources are individual deaths from malignant tumors reported by medical institutions from January 1,2018 to December 31,2022.Statistical analysis was conducted using indicators such as crude mortality rate,standardized mortality rate,composition ratio,annual percentage change(APC),PYLL,average years of life lost(AYLL).Results From 2018 to 2022,the crude mortality rate of malignant tumors in Taian city was 204.48/100000,and the standardized mortality rate was 104.69/100000.The deaths caused by malignant tumors accounted for 26.49%of all causes of death.The crude mortality rate of male malignant tumors was 272.06/100000,the standardized mortality rate was 150.26/100000;The crdue mortality rate for female malignant tumors was 136.48/100000,the standardized mortality rate was64.63/100000.The crude mortality rate of males was higher than that of females,and the difference was statistically significant(χ^(2)=6274.968,P<0.001).There was no statistical significance in the decline of standardized mortality of malignant tumors in the whole population,male and female,the APC values were-2.105(t=-2.931,P=0.061),-1.757(t=-2.651,P=0.077),and-2.319(t=-2.328,P=0.102),respectively.From the perspective of age trend,malignant tumors gradually increase after the age of 30,especially after the age of 50,and the crude mortality rate rises rapidly,and finally reaches a peak in the age group of≥85.The top five causes of death in male and female malignant tumors were consistent with those of the whole population,and they were lung cancer,esophageal cancer,stomach cancer,liver cancer,colorectal cancer in order.The top five causes of PYLL in male malignant tumors were the same as those of the whole population,which were lung cancer,liver cancer,esophageal cancer,stomach cancer and colorectal cancer
