摘　　要：功能性根治颈淋巴结清扫术是甲状腺癌根治手术的重要组成部分,在有效保护喉返神经和甲状旁腺功能的基础上可对肿瘤侧Ⅵ区淋巴结行预防性清扫,术前经病理证实颈侧区淋巴结转移时可行治疗性颈侧区淋巴结清扫。清扫时应注意清扫范围的规范化,包括Ⅱ、Ⅲ、Ⅳ、Ⅴb,Ⅱ(Ⅱb)、Ⅲ、Ⅳ区是颈侧区淋巴结清扫可接受的最小范围。同时要重视血管、肌肉、神经等功能保护,预防淋巴漏、出血、感染等并发症的发生。规范彻底的功能性根治颈淋巴结清扫是提高患者生活质量和生存期的重要保证。Functional radical cervical lymph node dissection is an important part of radical thyroid cancer surgery.On the basis of effective protection of recurrent laryngeal nerve and parathyroid function,preventive lymph node dissection in regionⅥcan be performed.When the preoperative pathology indicates suspected metastasis of lymph nodes in the lateral cervical region,lymph node dissection in the lateral cervical region can be performed.During the operation,attention should be paid to the standardized scope of dissection,which should include the zonesⅡ,Ⅲ,Ⅳ,and Vb.ZonesⅡ(Ⅱb),Ⅲ,andⅣare the minimum acceptable range for cervical lymph node dissection.We should pay attention to the protection of functions of blood vessels,muscles and to prevent the occurrence of complications such as lymphatic leakage,bleeding and infection.Standard and thorough neck lymph node dissection is an important guarantee to improve the quality of life and survival of patients.
