摘　　要：目的:研究VEGFR2、miR-27a-5p在胃癌组织中的表达与临床病理参数及预后的关系。方法:回顾性分析2020年1月至2022年12月98例行腹腔镜全胃切除术胃癌患者的病例资料。通过免疫组化法(IHC)和实时荧光定量PCR(qRT-PCR)检测胃癌组织及相应的癌旁组织(距癌组织边缘>5cm)中VEGFR2和miR-27a-5p mRNA的表达情况。数据使用软件SPSS 20.0进行统计学分析。miR-27a-5p表达水平等计量资料采用(x±s)表示,行独立样本t检验;临床病理参数等计数资料采用[例(%)]表示,行秩和检验或χ^(2)检验;采用Spearman秩相关分析胃癌组织中VEGFR2和miR-27a-5p表达的相关性;与患者预后的关系采用Kaplan-Meier法,并采用Log-Rank检验分析。P<0.05表示具有统计学差异。结果:胃癌组织中VEGFR2高表达率显著高于癌旁组织,胃癌组织中miR-27a-5p表达水平(1.08±0.42)显著低于癌旁组织(2.51±0.85),差异均有统计学意义(P<0.05),且两者表达呈负相关(r=-0.462,P=0.003)。VEGFR2的高表达与患者的病灶直径、T分期、分化程度、pTMN分期、脉管侵犯、淋巴结转移等明显相关(P<0.05),而miR-27a-5p的低表达却与患者肿瘤的分化程度、pTMN分期、脉管侵犯、淋巴结转移等显著相关(P<0.05)。通过Kaplan-Meier分析显示,VEGFR2高表达组的累积总生存率相较于VEGFR2低表达组(81.0%vs.94.7%)明显降低(P=0.047),而miR-27a-5p低表达组的累积总生存率相较于miR-27a-5p高表达组(81.7%vs.92.6%)明显降低(P=0.041)。结论:VEGFR2在胃癌组织中呈高表达,miR-27a-5p在胃癌组织中呈低表达,且两者之间可能存在相互作用,参与了胃癌的生长、增殖、分化、侵袭及迁移,从而影响胃癌的进程,有望成为胃癌诊治和预后评估的潜在靶点。Objective:To study the relationship between the expression of VEGFR2 and miR-27a-5p in gastric cancer tissues,clinicopathological parameters and prognosis.Methods:Data of 98 patients with gastric cancer who underwent laparoscopic total gastrectomy from January 2020 to December 2022 were retrospectively analyzed.Immunohistochemistry(IHC)and real-time quantitative fluorescent PCR(qRT-PCR)were used to detect the expression of VEGFR2 and miR-27a-5p mRNA in gastric cancer tissues and corresponding paracancer tissues(>5cm from the margin of cancer tissues).SPSS 20.0 software was used for statistical analysis.The expression level of miR-27a-5p and other quantitative data were expressed by(x±s)and independent sample t-test was performed.Clinical and pathological parameters and other statistical data were represented by[cases(%)],row Rank Sum test orχ^(2) test.Spearman Rank correlation was used to analyze the correlation between VEGFR2 and miR-27a-5p expression in gastric cancer tissues.The relationship with prognosis was analyzed by Kaplan-Meier method and Log-Rank test.P<0.05 indicates statistical difference.Results:The high expression rate of VEGFR2 in gastric cancer tissues was significantly higher than that in para-cancer tissues,and the expression level of miR-27a-5p in gastric cancer tissues(1.08±0.42)was significantly lower than that in para-cancer tissues(2.51±0.85),with statistical significance(P<0.05),and the expression of the two was negatively correlated(r=-0.462,P=0.003).The high expression of VEGFR2 was significantly correlated with lesion diameter,T stage,differentiation degree,pTMN stage,vascular invasion,and lymph node metastasis(P<0.05),while the low expression of miR-27a-5p was significantly correlated with tumor differentiation degree,pTMN stage,vascular invasion,and lymph node metastasis(P<0.05).Kaplan-Meier analysis showed that the cumulative overall survival rate of the VEGFR2 high expression group was significantly lower than that of the VEGFR2 low expression group(81.0%vs.94.7%)(P=0.047).The
