发疹性微静脉血管瘤

摘　　要：患者男,48岁,股部紫红色丘疹7年,加重1年。皮肤科检查:躯干和四肢散在多发数百个圆形、紫红色或暗红色的丘疹或斑丘疹,直径1~9mm,边界清晰,排列不规则,质韧,无压痛。皮损组织病理检查:真皮中薄壁、不规则分支的小血管浸润性增生,内皮无异型。免疫组化:增殖的血管内皮细胞中标志物CD31、CD34和Ⅷ因子相关抗原呈阳性。诊断:发疹性微静脉血管瘤。A 48-year-old male presented with multiple,scattered,asymptomatic,purplish or dark red papules or maculopapules measuring 1 mm to 9 mm in diameter on trunk and limbs.Hundreds of lesions were tough in texture,without tenderness,and irregularly arranged with clear borders.Purplish red papules on his thighs had existed for seven years and aggravated abruptly on trunk,arms and lower legs for one year.Histopathological examination showed infiltrative proliferation of thin-walled,rregularly branched,small blood vessels in the dermis without endothelial atypia.Immunohistochemical staining revealed that the endothelial cells of proliferating vessels were positive for CD31,CD34,and factorⅧ-related antigen.A diagnosis of eruptive microvenular hemangioma was made.
