摘　　要：目的探讨HHLA2与转录因子FOXO3A在非小细胞肺癌组织中表达水平及与其临床预后的关系。方法选取147例接受检查治疗的非小细胞肺癌患者作为研究对象,采用免疫组化法对癌组织及癌旁组织中HHLA2和FOXO3A表达进行测定。多因素cox回归分析非小细胞肺癌患者的预后相关因素。结果HHLA2在非小细胞肺癌组织中呈现高表达(57.1%vs 42.9%),FOX3OA蛋白阳性率低于癌旁组织(34.7%vs 65.3%)(P<0.05);HHLA2蛋白与TNM分期、淋巴结状态和复发显著相关,FOX3OA仅仅与TNM分期和淋巴结状态有关;单因素分析显示:TMN分期为Ⅲ~Ⅳ期、淋巴结转移、复发、HHLA2阳性和FOXO3A阴性与患者5年生存率相关;多因素分析显示:TMN分期为Ⅲ~Ⅳ期、淋巴结转移、HHLA2阳性、FOXO3A阴性是影响非小细胞肺癌患者预后的独立危险因素。HHLA2阴性、FOXO3A阳性的非小细胞肺癌患者5年总体生存率明显高于HHLA2阳性、FOXO3A阴性患者(P<0.05)。结论在非小细胞肺癌患者中,HHLA2蛋白表达升高与FOXO3A蛋白表达降低可能是影响其预后的独立危险因素。Objective To investigate the expression levels of HHLA2 and the transcription factor FOXO3A in non-small cell lung cancer(NSCLC)tissues and their relationship with clinical prognosis.Methods A total of 147 NSCLC patients who received examination and treatment were selected as the study subjects.The expression of HHLA2 and FOXO3A in cancer tissues and adjacent non-cancerous tissues was measured using immunohistochemistry.Multivariate Cox regression analysis was used to identify factors related to the prognosis of NSCLC patients.Results HHLA2 was highly expressed in NSCLC tissues(57.1%vs.42.9%),while the positive rate of FOXO3A protein was lower than that in adjacent tissues(34.7%vs.65.3%)(P<0.05).HHLA2 protein was significantly associated with TNM stage,lymph node status,and recurrence,while FOXO3A was only associated with TNM stage and lymph node status.Univariate regression analysis confirmed that TNM stageⅢ~Ⅳ,lymph node metastasis,recurrence,HHLA2 positive and FOXO3A negative were correlated with 5-year survival of patients;Multivariate regression analysis showed that TNM stageⅢ~Ⅳ,lymph node metastasis,HHLA2 positive and FOXO3A negative were independent risk factors affecting the prognosis of NSCLC patients.NSCLC patients who were HHLA2-negative and FOXO3A-positive had a significantly higher 5-year overall survival rate compared to HHLA2-positive and FOXO3A-negative patients(P<0.05).Conclusion Increased expression of HHLA2 and decreased expression of FOXO3A in NSCLC tissues may be independent risk factors affecting the prognosis of NSCLC patients.
