摘  要:目的研究脑脊液β-内啡肽(β-EP)、3-硝基酪氨酸(3-NT)水平与脑胶质瘤患者术后神经损伤程度及预后的关系。方法选择脑胶质瘤76例患者作为观察组,选择同期采集脑脊液标本非脑胶质瘤且无神经损伤的76例受试者作为对照组,检测两组脑脊液β-EP、3-NT水平,采用美国国立卫生研究院卒中量表(NIHSS)评估观察组患者神经损伤程度,并比较两组脑脊液β-EP、3-NT水平的差异,研究各因子与脑胶质瘤患者术后脑神经损伤程度及预后的关系。结果观察组患者脑脊液β-EP、3-NT水平均较对照组高(P<0.05);中度、重度脑胶质瘤患者脑脊液β-EP、3-NT水平较轻度高(P<0.05),重度脑胶质瘤患者脑脊液β-EP、3-NT水平较中度高(P<0.05);脑脊液β-EP、3-NT水平与脑胶质瘤患者神经损伤程度呈正相关(P<0.05),脑脊液β-EP表达与3-NT表达呈正相关(P<0.05);当脑脊液β-EP的截断值>42.03 ng/L时,其Youden指数为0.586,曲线下面积(AUC)为0.746,预测脑胶质瘤预后敏感度为66.67%,特异度为91.94%;当脑脊液3-NT的截断值>8.93μg/L时,其Youden指数为0.459,AUC为0.753,预测脑胶质瘤预后敏感度为55.56%,特异度为90.32%。结论脑胶质瘤患者脑脊液β-EP、3-NT水平随着神经损伤程度的加重而上升,且是造成不良预后的原因,有望作为评估脑胶质瘤进展的有效生物学标志物。Objective To study the relationship between cerebrospinal fluidβ-endorphin(β-EP)and 3-nitrotyrosine(3-NT)levels and postoperative neurological injury degree and prognosis in patients with glioma.Methods 76 patients with glioma were selected as the observation group,and 76 subjects without glioma and neurological injury in cerebrospinal fluid specimens were collected as the control group.The levels of cerebrospinal fluidβ-EP and 3-NT were detected in the 2 groups.National Institutes of Health Stroke Scale(NIHSS)was used to evaluate the degree of neurological injury in the observation group,and the differences in cerebrospinal fluidβ-EP and 3-NT levels were compared between the 2 groups,and the relationship between each factor and postoperative neurological injury degree and prognosis in patients with glioma was studied.Results The levels of cerebrospinal fluidβ-EP and 3-NT in the observation group were higher compared with those in the control group(P<0.05).The levels ofβ-EP and 3-NT in cerebrospinal fluid in patients with moderate and severe glioma were higher than those in patients with mild glioma(P<0.05),and the levels in patients with severe glioma were higher than those in patients with moderate glioma(P<0.05).The levels of cerebrospinal fluidβ-EP and 3-NT were positively correlated with the degree of neurological injury in patients with glioma(P<0.05),and cerebrospinal fluidβ-EP was positively correlated with 3-NT(P<0.05).When the cut-off value of cerebrospinal fluidβ-EP was>42.03 ng/L,the Youden index,area under the curve(AUC),sensitivity and specificity were 0.586,0.746,66.67%and 91.94%in predicting prognosis of glioma.When the cutoff value of cerebrospinal fluid 3-NT was>8.93μg/L,the Youden index,AUC,sensitivity and specificity were 0.459,0.753,55.56%and 90.32%respectively.Conclusion The levels ofβ-EP and 3-NT in cerebrospinal fluid of patients with glioma are increased with the severity of neurological injury,and the above 2 levels are the causes of poor prognosis,and may be effective biomark
