壳聚糖与透明质酸钠膀胱灌注治疗放射性膀胱炎的临床疗效比较

摘　　要：目的比较壳聚糖与透明质酸钠经膀胱灌注治疗放射性膀胱炎的临床疗效。方法选取2020年12月至2023年12月在江西省肿瘤医院就诊的62例放射性膀胱炎患者,按照1∶1随机分配的原则分为壳聚糖治疗组和透明质酸钠治疗组,每组各31例。壳聚糖治疗组在对症处理的基础上应用壳聚糖膀胱灌注治疗,透明质酸钠治疗组在对症处理的基础上应用透明质酸钠膀胱灌注治疗,2组患者均连续接受治疗3个月。观察治疗后2组患者血尿程度、膀胱刺激征程度、膀胱镜下表现、生活质量的变化,比较其效果。结果壳聚糖治疗组在降低血尿程度方面的有效率为93.55%,在缓解膀胱刺激征方面的有效率为90.91%,膀胱镜下表现有效率为93.55%,生活质量评分为(89.65±4.56);透明质酸钠治疗组在降低血尿程度方面的有效率为74.19%,在缓解膀胱刺激征方面的有效率为63.64%,膀胱镜下表现有效率为74.19%,生活质量评分为(84.15±5.93)。壳聚糖治疗组的各项评估指标均高于透明质酸钠治疗组(P<0.05)。结论壳聚糖较透明质酸钠在放射性膀胱炎的膀胱灌注治疗中更能降低血尿程度、缓解膀胱刺激症状、改善膀胱镜下表现、提升生活质量,应用价值高。Objective To compare the clinical efficacy of intravesical instillation of chitosan and sodium hyaluronate in the treatment of radiation cystitis.Methods A total of 62 patients with radiation cystitis treated were randomly divided into chitosan group(n=31)and sodium hyaluronate group(n=31).Chitosan treatment group was treated with chitosan bladder instillation and sodium hyaluronate treatment group was treated with sodium hyaluronate bladder instillation on the basis of symptomatic treatment.The patients in the 2 groups received continuous treatment for 3 months.The changes of hematuria,bladder irritation,cystoscopy and quality of life were observed and compared between the two groups after treatment.Results In the chitosan treatment group,the effective rates of hematuria,bladder irritation,cystoscopy and quality of life were 93.55%,90.91%,93.55%and(89.65±4.56),respectively.The effective rates of hematuria,bladder irritation,cystoscopy and quality of life in sodium hyaluronate group were 74.19%,63.64%,74.19%and(84.15±5.93)respectively.The evaluation index of chitosan group was higher than that of sodium hyaluronate group(P<0.05).Conclusion Chitosan is more valuable than sodium hyaluronate in reducing the degree of hematuria,relieving the symptoms of bladder irritation,improving cystoscopic performance,and enhancing the quality of life in the bladder perfusion treatment of radiation cystitis.
