摘　　要：目的探讨内镜微创手术对早期食管癌术后免疫功能及复发率的影响。方法随机将接受治疗的112例早期食管癌患者分为对照组与观察组,每组各56例,对照组行内镜下黏膜切除术(EMR),观察组行内镜黏膜下剥离术(ESD),观察两组的手术相关指标、免疫功能(CD3^(+)、CD4^(+)、CD8^(+))、并发症和复发率。结果与对照组比较,观察组手术时间明显更短,术中出血量明显更少(P<0.05);观察组术后CD3^(+)、CD4^(+)水平均高于对照组,CD8^(+)低于对照组(P<0.05);并发症发生率比较,观察组(5.36%)明显较对照组(19.64%)低(P<0.05);术后12个月复发率比较,观察组(5.36%)明显低于对照组(17.86%)(P<0.05)。结论对早期食管癌患者开展ESD治疗,出血量较少,手术时间更短,不仅降低患者的术后并发症发生率和复发风险,还能减轻对患者免疫功能的影响。Objective To investigate the effects of endoscopic minimally invasive surgery on immune function and recurrence rate of early esophageal cancer.Methods 112 patients with early esophageal cancer were randomly divided into the control group and the observation group,56 cases in each group.The control group underwent endoscopic mucosal resection(EMR),and the observation group underwent endoscopic submucosal dissection(ESD).The operation-related indexes,immune function(CD3^(+),CD4^(+),CD8^(+)),complications and recurrence rate of the 2 groups were observed.Results Compared with the control group,the operation time in the observation group was significantly shorter and the amount of blood loss was significantly less(P<0.05).The levels of CD3^(+)and CD4^(+)in the observation group were higher than those in the control group,and CD8^(+)were lower than those in the control group(P<0.05).The incidence of complications in the observation group(5.36%)was significantly lower than that in the control group(19.64%)(P<0.05).The recurrence rate in the observation group(5.36%)was significantly lower than that in the control group(17.86%)(P<0.05).Conclusion ESD therapy for early esophageal cancer patients has less blood loss,shorter operation time,not only can reduce the incidence of postoperative complications and recurrence risk,but also reduce the impact on the immune function of patients.
