肿瘤患者放疗依从性的影响因素及对策研究

摘　　要：目的分析肿瘤患者的放疗依从性影响因素,针对性提出应对策略,提升治疗效果。方法选择2022年1月—2023年3月在本院接受放射治疗的180例肿瘤患者作为研究对象,利用放疗依从性评分表进行调查,整理患者一般临床资料,分析放疗依从性的影响因素及与评分的相关性。结果配偶、陪护、付费方式、月收入、疼痛分级、行动能力、放疗部位、放疗次数的依从性评分比较,组间差异均具有统计学意义(P<0.05)。多因素logistic回归分析显示,无配偶、无陪护、自费、月收入<3000元、疼痛≥Ⅱ度、需轮椅辅助、腹盆部肿瘤、三次及以上放疗为依从性评分的独立危险因素(P<0.05),与肿瘤放射治疗依从性评分呈负相关(P<0.05)。结论肿瘤患者依从性评分整体较低,影响因素众多且个体差异明显,临床治疗和护理时应采取相应针对措施,弱化影响,提升治疗依从性。Objective To analyze the influencing factors of radiotherapy compliance among tumor patients,and put forward countermeasures to improve the therapeutic effects.Methods 180 tumor patients having received radiotherapy in our hospital during January 2022 and March 2023 were selected as study subjects.The radiotherapy compliance rating scale was used for investigation.General clinical data of patients were sorted out and the influencing factors of radiotherapy compliance and its correlation with score were analyzed.Results There were statistically significant differences in the compliance ratings among groups,including spouse,carer,payment method,monthly income,pain grade,action capability,radiotherapy site and radiotherapy times(P<0.05).Multivariate logistic regression analysis showed that no spouse,no chaperonage,self-payment,monthly income less than 2000 yuan,pain over or equal toⅡdegree,wheelchair assistance,abdominal pelvic tumor,three or more radiotherapy were independent risk factors for compliance score(P<0.05),and were negatively correlated with the compliance score of tumor radiotherapy(P<0.05).Conclusions The overall compliance score of tumor patients is low,with numerous influencing factors and obvious individual differences.Corresponding measures should be taken in clinical treatment and nursing to weaken the influence and improve treatment compliance.
