摘　　要：目的基于力学研究缝匠肌在偏瘫步态下肢外旋中的作用机制。方法选取2022年6月至2023年6月于我院康复治疗科就诊的60例康复患者作为研究对象,采用随机数字表法将其分为对照组和治疗组,每组30例。对照组给予常规训练,治疗组在常规训练的基础上增加缝匠肌牵伸训练及抑制训练。比较两组的足偏角以及患侧下肢缝匠肌肌力、肌张力。结果治疗后,两组的足偏角均小于治疗前,且治疗组明显小于对照组,差异具有统计学意义(P<0.05)。治疗前、后,两组的缝匠肌肌力比较,差异均无统计学意义(P>0.05)。治疗后,治疗组的缝匠肌肌张力优于对照组,差异具有统计学意义(P<0.05)。结论在偏瘫步态训练中降低缝匠肌张力可以改善患者下肢外旋的角度。Objective To study the mechanism of sartorius muscle in lower limb external rotation during hemiplegic gait based on mechanics.Methods From June 2022 to June 2023,60 rehabilitation patients who were treated in the rehabilitation treatment department of our hospital were selected as the research objects,and the patients were divided into control group and treatment group by random number table method,with 30 cases in each group.The control group was given routine training,and the treatment group was given sartorius muscle stretching training and inhibition training on the basis of routine training.The foot deviation angle,sartorius muscle strength and muscle tension of the affected lower limbs were compared between the two groups.Results After treatment,the foot deviation angle of the two groups was smaller than that before treatment,that in the treatment group was significantly smaller than the control group,and the differences were statistically significant(P<0.05).Before and after treatment,there was no significant difference in sartorius muscle strength between the two groups(P>0.05).After treatment,the sartorius muscle tension in the treatment group was better than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Reducing the sartorius muscle tension in hemiplegic gait training can improve the external rotation angle of lower limb.
