产前诊断胎儿唇腭裂畸形的MRI和超声对比研究

摘　　要：目的:探讨MRI对胎儿唇腭裂畸形超声诊断的补充价值。方法:2021年1月至2021年12月在我院经产前超声诊断胎儿唇裂(并或不并腭裂)17例。超声行完整的常规胎儿检查外并行颌面部超声检查。超声检查后3天内行首次胎儿MRI检查。将超声、MRI和产后随访结果进行对照分析。结果:超声正确诊断12/17例唇腭裂胎儿,其中3例合并中枢神经系统畸形;MRI最终正确诊断17/17例唇腭裂胎儿(4例首次MRI检查漏诊,随访至晚孕期均得到正确诊断),其中3例合并中枢神经系统异常,纠正2例超声误诊的合并中枢神经系统畸形。补充诊断2例合并腭裂的超声漏诊病例,纠正1例超声误诊病例。结论:对胎儿唇腭裂的产前诊断,MRI可为超声提供有价值的补充信息,尤其是合并中枢神经系统畸形患者。Objective:To investigate the complementary value of MRI in the ultrasonic diagnosis of fetal cleft lip and palate.Methods:From January 2021 to December 2021,17 cases of fetal cleft lip(with or without cleft palate)were diagnosed by prenatal ultrasonography in our hospital.A complete routine fetal examination was performed and maxllofacial ultrasound was performed.The first fetal MRI examination was performed within 3 days after ultrasound.The results of ultrasound,MRI and postpartum follow-up were analyzed.Results:The ultrasonography correctly diagnosed 12/17 cases of cleft lip and palate fetuses,of which 3 cases were complicated with central nervous system malformations.MRI finally correctly diagnosed 17/17 cases of cleft lip and palate fetuses(4 cases missed the first MRI examination,and were correctly diagnosed until late pregnancy),of which 3 cases were complicated with central nervous system abnormalities,and 2 cases were corrected by ultrasound misdiagnosis with central nervous system abnormalities.Two cases of missed ultrasound diagnosis complicated with cleft palate were diagnosed,and one case of misdiagnosis was corrected.Conclusion:MRI can provide valuable supplementary information for ultrasound in prenatal diagnosis of fetal cleft lip and palate,especially in patients with central nervous system malformations.
