摘 要:目的探讨鼻咽癌(NPC)患者血清SMAD家族成员4(SMAD4)、组织多肽特异性抗原(TPS)表达水平及与临床特征和预后的关系。方法选取43例NPC患者作为NPC组,40例健康体检者作为健康组,比较两组受试者血清SMAD4、TPS水平,分析SMAD4、TPS与NPC患者临床特征的关系。采用Cox回归模型分析NPC患者预后的影响因素。结果NPC组患者血清SMAD4水平明显低于健康组,TPS水平明显高于健康组,差异均有统计学意义(P﹤0.01)。不同临床分期、颈淋巴结转移情况、颅神经侵犯情况NPC患者血清SMAD4、TPS水平比较,差异均有统计学意义(P﹤0.05)。Cox回归分析结果显示,有颅神经侵犯、血清SMAD4水平﹤97.43 ng/L、血清TPS水平≥164.44 U/L均为NPC患者预后不良的独立危险因素(P﹤0.01)。结论NPC患者血清SMAD4呈异常低表达,TPS呈异常高表达,其水平与NPC患者的临床特征有关,且与预后相关。Objective To investigate the expression levels of serum SMAD family member 4(SMAD4)and tissue polypeptide specific antigen(TPS)in patients with nasopharyngeal carcinoma(NPC)and their relationship with clinical characteristics and prognosis.Method A total of 43 NPC patients were selected as NPC group and 40 healthy subjects were selected as healthy group.Serum levels of SMAD4 and TPS were compared between the two groups,and the relationship between SMAD4,TPS and clinical characteristics of NPC patients were analyzed.The influencing factors of the prognosis of NPC patients were analyzed by Cox regression model.Result The level of serum SMAD4 in NPC group was significantly lower than that in healthy group,and the level of TPS was significantly higher than that in healthy group,the differences were statistically significant(P<0.01).Serum SMAD4 and TPS levels in NPC patients with different clinical stages,cervical lymph node metastasis and cranial nerve invasion were compared,the differences were statistically significant(P<0.05).Cox regression analysis showed that cranial nerve invasion,serum SMAD4 level<97.43 ng/L,serum TPS level≥164.44 U/L were independent risk factors for poor prognosis in NPC patients(P<0.01).Conclusion The serum SMAD4 expression is abnormally low and TPS expression is abnormally high in NPC patients,which are related to the clinical characteristics and prognosis of NPC patients.
