摘　　要：目的探究血清长链非编码RNA肺癌相关转录本1(lncRNA LUCAT1)、微RNA-514b-3p(miR-514b-3p)、甲胎蛋白(AFP)表达水平对预测早期肝细胞癌(HCC)病人根治术后复发的临床价值。方法选取2016年3月至2019年6月西安市中心医院诊治的130例Ⅰ~Ⅱ期HCC病人为研究对象,对所有早期HCC病人随访3年,按其行根治术后是否复发分为未复发组(n=92)和复发组(n=38)。检测并比较复发组、未复发组早期HCC病人血清lncRNA LUCAT1、miR-514b-3p、AFP水平;受试者操作特征曲线(ROC曲线)分析血清lncRNA LUCAT1、miR-514b-3p、AFP对早期HCC病人根治术后复发的预测价值;多因素logistic回归分析早期HCC病人根治术后复发的影响因素;Pearson法分析根治术后复发的早期HCC病人血清lncRNA LUCAT1表达水平与miR-514b-3p的关系。结果与未复发组[0.99±0.33、(17.42±3.67)μg/L、1.01±0.34]相比,复发组早期HCC病人血清lncRNA LUCAT1(1.87±0.63)、AFP水平(25.38±5.18)μg/L升高(P<0.05),miR-514b-3p水平(0.56±0.19)降低(P<0.05)。血清lncRNA LUCAT1、miR-514b-3p、AFP预测早期HCC病人根治术后复发的曲线下面积(AUC)分别为0.81、0.68、0.83,且血清lncRNA LUCAT1、AFP联合预测早期HCC病人根治术后复发的AUC为0.93,灵敏度为93.8%,特异度为84.8%。血清lncRNA LUCAT1、miR-514b-3p联合预测AFP阴性病人根治术后复发的AUC为0.91,高于二者单独预测(P<0.05);血清lncRNA LUCAT1、miR-514b-3p单独及联合预测AFP阳性病人根治术后复发的AUC与AFP单独预测相比,差异无统计学意义(P>0.05)。AFP、lncRNA LUCAT1是影响早期HCC病人根治术后复发的独立危险因素(P<0.05),miR-514b-3p是独立保护因素(P<0.05)。根治术后复发的早期HCC病人血清lncRNA LUCAT1水平与miR-514b-3p水平呈负相关(P<0.05),且lncRNA LUCAT1与miR-514b-3p存在结合位点。结论根治术后复发的早期HCC病人血清lncRNA LUCAT1水平较高,miR-514b-3p水平较低,血清lncRNA LUCAT1与AFP联合检测更有利于临床预�Objective To investigate the clinical value of serum long noncoding RNA lung cancer-related transcript 1(lncRNA LUCAT1),microRNA-514b-3p(miR-514b-3p)and alpha-fetoprotein(AFP)expression levels in predicting the recurrence of early hepatocellular carcinoma(HCC)patients after radical surgery.Methods A total of 130 patients with stageⅠ-ⅡHCC who were diagnosed and treated at Xi'an Central Hospital between March 2016 and June 2019 were selected as the study subjects,and all the early HCC patients who were followed up for 3 years were divided into the nonrecurrent group(n=92)and the recurrent group(n=38)according to whether or not they had relapsed after undergoing radical surgery.The serum levels of the lncRNA LUCAT1,miR-514b-3p and AFP in the recurrent and nonrecurrent groups of early HCC patients were detected and compared;receiver operating characteristic(ROC)curves were used to analyze the predictive value of the serum lncRNA LUCAT1,miR-514b-3p and AFP for the recurrence of early HCC patients after radical surgery.Multivariate logistic regression analysis was made of the factors influencing recurrence after radical surgery in early-stage HCC patients.Pearson's method was applied to analyze the relationship between the serum lncRNA LUCAT1 expression level and miR-514b-3p in early HCC patients who experienced recurrence after radical surgery.Results Compared with those in the nonrecurrent group[0.99±0.33,(17.42±3.67)μg/L,1.01±0.34],early HCC patients in the recurrent group had increased serum lncRNA LUCAT11.87±0.63 and AFP levels(25.38±5.18)μg/L and decreased miR-514b-3p level 0.56±0.19(P<0.05).The ar‐eas under the curves(AUCs)of the serum lncRNA LUCAT1,miR-514b-3p and AFP for predicting postradical recurrence in early HCC patients were 0.81,0.68 and 0.83,respectively,and the AUCs of the combination of the serum lncRNA LUCAT1 and AFP for predicting postradical recurrence in early-stage HCC patients was 0.93,with a sensitivity of 93.8%and a specificity of 84.8%.The AUC of the combination of the serum ln
