摘 要:目的探讨Toll样受体4(TLR4)、环氧合酶2(COX⁃2)在乳腺癌组织内的表达及对微血管生成的影响。方法选取2021年4月至2022年4月南充市中心医院收治的乳腺癌患者60例,经手术治疗后,取患者癌组织及癌旁正常组织标本,采用免疫组织化学法检测组织中的COX⁃2、TLR4表达情况及微血管密度(MVD),分析乳腺癌术后组织中影响COX⁃2、TLR4表达的因素及其与微血管生成的关系。结果癌组织中COX⁃2、TLR4表达阳性率及MVD水平高于癌旁正常组织(P<0.05);单因素分析显示,TLR4、COX⁃2表达阳性乳腺癌组织中肿瘤长径>2 cm占比、临床分期Ⅲ~Ⅳ期占比、淋巴结转移占比均高于表达阴性癌组织(P<0.05);多因素Logistic回归分析显示,肿瘤长径>2 cm、临床分期Ⅲ~Ⅳ期、淋巴结转移是导致乳腺癌术后组织出现COX⁃2、TLR4表达阳性的危险因素(P<0.05);TLR4、COX⁃2表达阳性的乳腺癌组织MVD水平高于表达阴性的癌组织(P<0.05)。结论COX⁃2、TLR4在乳腺癌组织内高水平表达与微血管生成有着密切关系,可促进疾病发展。Objective To investigate the expression of Toll⁃like receptor 4(TLR4)and cyclooxygenase⁃2(COX⁃2)in the tissue of breast cancer after surgery and their effects on angiogenesis.Methods Totally 60 patients with breast cancer treated in the Nanchong Central Hospital from April 2021 to April 2022 were selected as research subjects.After surgical treatment,the samples of cancer tissues and normal tissue adjacent to the cancer were taken for study.The expression of COX⁃2,TLR4 and microvessel density(MVD)in the tissues were detected by immunohistochemical method.The factors affecting the expression of COX⁃2,TLR4 and their relationship with angiogenesis in the tissues of breast cancer after surgery were analyzed.Results The positive expression of COX⁃2,TLR and MVD in the breast cancer tissue was significantly higher than those in the adjacent normal tissue(P<0.05).Univariate analysis revealed that the proportions of tumor diameter>2 cm,clinical stage III⁃IV lymph and node metastasis in the breast cancer tissues were significantly higher than those in the negative expression(P<0.05).Multivariate Logistic regression analysis showed that tumor diameter>2 cm,clinical stage III⁃IV and lymph node metastasis were the risk factors for the positive expression of COX⁃2 and TLR4 in the tissue of breast cancer after surgery(P<0.05).The MVD level of TLR4 and COX⁃2 positive expression in breast cancer tissue was significantly higher than that of the negative expression(P<0.05).Conclusion The high expression levels of COX⁃2 and TLR4 in the breast breast cancer tissue after surgery are closely related to angiogenesis,which might promote the development of the disease.
