磁共振T2加权像、动态增强扫描序列参数在前列腺癌中的表现及与前列腺特异性抗原水平、国际泌尿病理学会分级的相关性

摘　　要：目的探讨磁共振T2加权像(T2WI)、磁共振动态增强扫描(DCE⁃MRI)参数在前列腺癌中的表现及与前列腺特异性抗原(PSA)、国际泌尿病理学会(ISUP)分级的相关性。方法选取2019年3月至2023年6月在宜兴市中医医院就诊的82例前列腺病变患者作为研究对象,其中前列腺癌患者52例,前列腺增生患者30例,比较不同患者T2WI、DCE⁃MRI序列参数。结果前列腺癌患者T2WI表现≥4分的比例为50.00%,高于前列腺增生患者(P<0.05);前列腺癌患者峰值时间(T_(max))为70541.44(45035.20,90655.41)ms,短于前列腺增生患者(P<0.05),而最快强化率(R_(max))为(36.60±14.41),高于前列腺增生患者(P<0.05)。Ⅲ~Ⅳ期、ISUP分级≥4级、PSA水平≥50 ng/ml的前列腺癌患者T2WI表现≥4分的比例分别为62.50%、75.00%和63.16%,高于Ⅰ~Ⅱ期、ISUP分级≤3级和PSA水平<50 ng/ml患者(P<0.05);Ⅲ~Ⅳ期的前列腺癌患者T_(max)为68405.44(43506.43,82204.32)ms,短于Ⅰ~Ⅱ期患者(P<0.05),而R_(max)为(39.16±9.50),高于Ⅰ~Ⅱ期患者(P<0.05);ISUP分级≥4级的前列腺癌患者T_(max)为66504.32(43506.43,84053.12)ms,短于ISUP分级≤3级患者(P<0.05),而R_(max)为(40.38±9.75),高于ISUP分级≤3级患者(P<0.05);PSA水平≥50 ng/ml的患者T_(max)为63044.22(45035.20,82204.32)ms,短于PSA水平<50 ng/ml的患者(P<0.05),而R_(max)为(39.15±9.05),高于PSA水平<50 ng/ml的患者(P<0.05);T_(max)与PSA呈负相关(P<0.05),R_(max)与PSA呈正相关(P<0.05);T2WI表现和R_(max)与ISUP分级呈正相关(P<0.05),T_(max)与ISUP分级呈负相关(P<0.05)。结论T2WI、DCE⁃MRI序列参数与前列腺癌临床分期、ISUP分级、PSA水平存在相关性,值得临床进一步研究。Objective To explore the presentation of T2 weighted magnetic resonance imaging(T2WI)and dynamic contrast⁃enhanced magnetic resonance imaging(DCE⁃MRI)parameters in patients with prostate cancer and their correlations with prostate specific antigen(PSA)level and International Society of Urological pathology(ISUP)grading.Methods A total of 82 patients with prostate diseases who were admitted to Yixing Traditional Chinese Medicine Hospital between March 2019 and June 2023 were selected as research objects,including 52 patients with prostate cancer and 30 patients with benign prostatic hyperplasia.T2WI and DCE⁃MRI parameters of these patients were compared.Results The proportion of prostate cancer patients with T2WI score≥4 was 50.00%,which was higher than that of the benign prostatic hyperplasia patients(P<0.05).The peak time(T_(max))of prostate cancerpatients was 70541.44(45035.20,90655.41)ms,which was shorter than that of the patients with benign prostatic hyperplasia(P<0.05).The fastest enhancement rate(R_(max))of prostate cancer patients was 36.60±14.41,which was higher than that in patients with benign prostatic hyperplasia(P<0.05).The proportions of patients with T2WI score≥4 in stagesⅢ-Ⅳ,ISUP grading≥4,and PSA≥50 ng/ml were 62.50%,75.00%and 63.16%,respectively,which were significantly higher than those in stagesⅠ-Ⅱ,ISUP grading≤3,and PSA level<50 ng/ml(all P<0.05).The T_(max)of patients in stagesⅢ-Ⅳwas 68405.44(43506.43,82204.32)ms,which was lower than that of stagesⅠ-Ⅱpatients(all P<0.05).The R_(max)of patients in stagesⅢ-Ⅳwas 39.16±9.50,which was higher than that of stagesⅠ-Ⅱpatients(all P<0.05).The T_(max)of patients with ISUP grading≥4 was 66504.32(43506.43,84053.12)ms,which was lower than that of patients with ISUP grading≤3(P<0.05).The R_(max)of patients with ISUP grading≥4 was 40.38±9.75,which was higher than that of patients with ISUP grading≤3(P<0.05).The T_(max)of patients with PSA≥50 ng/ml was 63044.22(45035.20,82204.32)ms,which was shorter than
