摘　　要：目的:观察自拟通络益胃汤辅助XELOX化疗方案治疗胃癌患者的效果。方法:选取我院2021年5月—2023年8月就诊的胃癌患者82例作为研究对象,随机分为对照组、观察组,各41例。对照组给予XELOX方案化疗及保肝、护胃等西医辅助治疗,观察组在此基础上给予联合经络学说指导下自拟通络益胃汤治疗,比较两组中医证候评分、免疫功能、肿瘤标记物水平、胃肠激素变化及毒副反应发生率。结果:治疗后,观察组中医证候积分低于对照组(P<0.05);观察组CEA、CA199以及CA724水平低于对照组(P<0.05);观察组CD4^(+)、CD4^(+)/CD8^(+)水平以及NK细胞含量高于对照组,CD8^(+)低于对照组(P<0.05);观察组胃肠激素水平高于对照组,血管活性肠肽低于对照组(P<0.05);两组白细胞异常、神经毒性、血小板减少发生率比较,差异无统计学意义(P>0.05),而观察组恶心呕吐发生率低于对照组(P<0.05)。结论:自拟通络益胃汤联合XELOX化疗方案治疗胃癌患者疗效确切,能有效改善临床症状,减轻胃肠道不良反应,降低肿瘤标志物水平,提升免疫功能,改善胃肠激素水平,且不增加神经系统、血液等其他毒副反应发生风险。Objective:To observe the effect of self-made Tongluo Yiwei decoction combined with XELOX chemotherapy in the treatment of patients with gastric cancer.Methods:A total of 82 patients with gastric cancer who were treated in our hospital from May 2021 to August 2023 were randomly divided into control group and observation group,with 41 patients in each group.The control group was treated with XELOX chemotherapy and western medicine adjuvant therapy such as liver protection and stomach protection.On this basis,the observation group was treated with self-made Tongluo Yiwei Decoction under the guidance of combined meridian theory.The TCM syndrome score,immune function,tumor marker level,gastrointestinal hormone changes and the incidence of toxic and side effects were compared between the two groups.Results:After treatment,the scores of TCM syndromes in the observation group were lower than those in the control group(P<0.05);the levels of CEA,CA199 and CA724 in the observation group were lower than those in the control group(P<0.05);the levels of CD4^(+),CD4^(+)/CD8^(+)and NK cells in the observation group were higher than those in the control group,and CD8^(+)was lower than that in the control group(P<0.05);the level of gastrointestinal hormones in the observation group was higher than that in the control group,and the vasoactive intestinal peptide was lower than that in the control group(P<0.05).There was no significant difference in the incidence of leukocyte abnormality,neurotoxicity and thrombocytopenia between the two groups(P>0.05),while the incidence of nausea and vomiting in the observation group was lower than that in the control group(P<0.05).Conclusion:Self-made Tongluo Yiwei decoction combined with XELOX chemotherapy is effective in the treatment of gastric cancer patients,which can effectively improve clinical symptoms,reduce gastrointestinal adverse reactions,reduce the level of tumor markers,enhance immune function,improve gastrointestinal hormone levels,and do not increase the risk of nervous system,blo
