摘 要:目的探讨维生素B6联合鼠神经生长因子治疗难治性癫痫(refractory epilepsy,RE)患儿的疗效。方法选取RE患儿89例,根据住院号随机分为观察组(n=43)和对照组(n=46)。对照组在原抗癫痫药物治疗基础上联合应用左乙拉西坦片,观察组在对照组基础上辅助应用维生素B6联合鼠神经生长因子治疗。比较两组患儿临床疗效,治疗前后外周血miR-99a-5p、miR-125b-5p、泛素羧基末端水解酶L1(ubiquitin C-terminal hydrolase-L1,UCH-L1)水平变化和对脑电活动的影响,评估患儿智力水平和生活质量改善情况。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组miR-99a-5p、miR-125b-5p、UCH-L1和θ波水平,均明显低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组语言智商、操作智商和总智商评分,儿童癫痫生活质量量表(quality of life in childhood epilepsy,QOLCE)总分,均明显高于对照组,差异有统计学意义(P<0.05)。结论维生素B6联合鼠神经生长因子治疗难治性癫痫效果较佳,可显著降低患儿外周血miR-99a-5p、miR-125b-5p、UCH-L1水平,控制脑电活动,从而改善患儿智力水平和生活质量。Objective To investigate the efficacy of vitamin B6 combined with mouse nerve growth factor in the treatment of children with refractory epilepsy(RE).Methods A total of 89 children with RE were randomly divided into observation group(n=43)and control group(n=46)based on their hospitalization numbers.The control group received levetiracetam tablets on the basis of the original antiepileptic drugs,while the observation group was additionally treated with vitamin B6 combined with mouse nerve growth factor.The clinical efficacy,changes in peripheral blood levels of miR-99a-5p,miR-125b-5p,and ubiquitin C-terminal hydrolase-L1(UCH-L1)after treatment,effects on electroencephalographic activity,and improvements in intellectual level and quality of life were compared between the two groups.Results The overall response rate in the observation group was higher than that in the control group,with a statistically significant difference(P<0.05).After treatment,the levels of miR-99a-5p,miR-125b-5p,UCH-L1,and theta wave in the observation group were significantly lower than those in the control group,with statistically significant differences(P<0.05).After treatment,the scores of verbal IQ,performance IQ,and full-scale IQ,as well as the total score of the Quality of Life in Childhood Epilepsy(QOLCE)scale,were significantly higher in the observation group than in the control group,with statistically significant differences(P<0.05).Conclusions The treatment of RE with vitamin B6 combined with mouse nerve growth factor shows good efficacy.It can significantly reduce the levels of miR-99a-5p,miR-125b-5p,and UCH-L1 in peripheral blood,control electroencephalographic activity,and thereby improve the intellectual level and quality of life of children with RE.
