应用复方托吡卡胺滴眼液辅助诊断霍纳综合征一例

摘　　要：本文报道了一例因左眼上睑下垂就诊于眼科的霍纳综合征患者,该患者曾行左侧颈部神经鞘瘤切除术。予患者左眼复方托吡卡胺滴眼液,5 min后检查患者睑裂高度变化,发现患者左眼下垂的上睑上抬。该病例提示当患者因上睑下垂就诊于眼科时,眼科医生应仔细询问患者病史,全面进行眼科相关查体,并进行鉴别诊断,从而及时发现并治疗患者原发病。同时,也提示在初步考虑因上睑下垂就诊的患者为霍纳综合征时,可先尝试使用复方托吡卡胺滴眼液来观察患者的上睑变化以支持诊断或进行进一步检查。This paper reports a case of Horner syndrome manifested as ptosis of the left eye in ophthalmology clinic,which had undergone resection of schwannoma in the left neck.Compound tropicamide eye drops were given to the patient's left eye.After 5 minutes,the upper eyelid of the patient's left eye was raised.This case suggests that when the patient is treated in ophthalmology clinic due to ptosis,the ophthalmologist should carefully ask the patient’s medical history,conduct a comprehensive eye-related physical examination,and make differential diagnosis,so as to detect and treat the patient's primary disease in time.When the physician initially consider the patient with ptosis as Horner syndrome,it is suggested to use compound tropicamide eye drops to observe the changes of patient's upper eyelid to support the diagnosis or further examination.
