谷精草合剂加减治疗小儿多发性抽动症肝旺脾虚证临床观察

摘　　要：目的观察谷精草合剂加减治疗小儿多发性抽动症肝旺脾虚证临床疗效。方法将100例小儿多发性抽动症肝旺脾虚证患儿按照随机数字表法分为2组,对照组50例予盐酸硫必利片治疗,治疗组50例予谷精草合剂加减治疗,2组均持续治疗4周为1个疗程,共治疗2个疗程。比较2组治疗前后耶鲁抽动严重程度量表(YGTSS)评分、中医症状评分变化;比较2组治疗前后免疫指标免疫球蛋白A(IgA)、IgG、IgM水平变化;比较2组治疗前后微量元素锌(Zn)、钙(Ca)水平变化。结果2组治疗后运动性抽动、发声性抽动、综合性损害评分及总评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后中医症状主症、次症评分及总评分均较本组治疗前降低(P<0.05),且治疗组降低更明显(P<0.05)。2组治疗后IgA、IgG、IgM水平均较本组治疗前升高(P<0.05),且治疗组升高更明显(P<0.05)。2组治疗后Zn、Ca水平均较本组治疗前升高(P<0.05),且治疗组升高更明显(P<0.05)。结论谷精草合剂加减治疗小儿多发性抽动症肝旺脾虚证,在改善患儿抽动症状的同时可有效提高机体免疫功能和微量元素水平。Objective To observe the clinical efficacy of modified Gujingcao mixture for children with Tourette's syndrome(TS)of liver exuberant and spleen weakness.Methods One hundred patients with TS of liver exuberant and spleen weakness were randomly assigned to receive either tiapride hydrochloride tablets(the control group,n=50)or modified Gujingcao mixture(the treatment group,n=50).The treatment continued for 2 courses(1 course for 4 weeks),the aim was to compare Yale Global Tic Severity Scale(YGTSS),traditional Chinese medicine(TCM)symptom score,immunoglobulin A(IgA),IgG,IgM,zinc(Zn 2+),calcium(Ca 2+).Results After treatment,the domain scores(including sports tic,vocal tic and integrated damage)and total scores of YGTSS in the both groups were significantly decreased(P<0.05),which decreased notably in the treatment group compared with the control group(P<0.05).the domain scores(main symptom and secondary symptom)and total scores of TCM symptom and in the both groups were significantly decreased(P<0.05),which were significantly lower in the treatment group than in the control group(P<0.05).IgA,IgG and IgM in the both groups after treatment were significantly increased(P<0.05),the increase in the treatment group was more common than the control group(P<0.05).Zn 2+and Ca 2+in the both groups were significantly increased(P<0.05),the increase in the treatment group was more prevalent compared with the control group(P<0.05).Conclusion For TS patients of liver exuberant and spleen weakness,modified Gujingcao mixture can not only improve tic symptoms,but also improve the body's immune function,it can effectively promote the absorption of trace elements by improving the symptoms of spleen deficiency.
