老年脑梗死后遗症鼻饲患者鼻饲管拔管原因分析

摘　　要：目的了解老年医学科脑梗死后遗症鼻饲患者鼻饲管的拔管率和现状,探讨老年脑梗死后遗症鼻饲患者鼻饲管拔管的独立影响因素。方法采取整群抽样的方法,选取2020年1月至2022年12月福建省老年医院老年医学科收治的脑梗死后遗症鼻饲患者300例为研究对象,回顾性调查脑梗死后遗症鼻饲患者的病历,查找鼻饲管拔管的影响因素,分析其独立影响因素。结果调查的300例脑梗死后遗症鼻饲患者鼻饲管非计划拔管率为19.00%。单因素分析显示:年龄、意识障碍、日常生活活动能力(ADL)评分、移动能力、消化道是否完整、吞咽功能,利手肌力、利手灵活度是脑梗死后遗症鼻饲患者鼻饲管拔管的影响因素。Logistic回归分析显示:ADL评分、吞咽功能1~4级、意识障碍、移动能力、消化道是否完整是脑梗死后遗症鼻饲患者鼻饲管拔管的独立影响因素。结论影响脑梗死后遗症鼻饲患者鼻饲管拔管的因素有很多,其中,独立影响因素有意识障碍、ADL评分、移动能力、消化道是否完整、吞咽功能1~4级。Objective To investigate the extubation rate and status of nasal feeding tube in patients with cerebral infarction sequela in geriatric medicine department,and to explore the independent influencing factors of extubation of nasal feeding tube in patients with cerebral infarction sequela.Methods A cluster sampling method was used to select 300 cases of nasogastric patients with cerebral infarction sequela treated in the geriatric medicine Department of Fujian Geriatric Hospital from January 2020 to December 2022 as the research objects.The medical records of patients with cerebral infarction sequela were retrospectively investigated,and the influencing factors of nasogastric tube extraction were found and their independent influencing factors were analyzed.Results The unplanned extubation rate was 19.00%in 300 patients with cerebral infarction sequela.Univariate analysis showed that age,disturbance of consciousness,activity of daily living(ADL)score,mobility,AGI grade,digestive integrity,swallowing function,hand muscle strength and hand flexibility were the influencing factors of nasogastric tube extraction in patients with cerebral infarction sequela.Logistic regression analysis showed that ADL score,swallowing function grades 1 to 4,consciousness disturbance,mobility ability and integrity of digestive tract were independent influencing factors for nasogastric tube extraction in patients with cerebral infarction sequela.Conclusions There are many factors affecting nasogastric tube extubation in patients with cerebral infarction sequela.Among them,there are independent factors such as disturbance of consciousness,ADL score,mobility,integrity of digestive tract and swallowing function grades 1 to 4.
