活动和固定义齿修复在牙周病伴牙列缺损患者治疗中的应用效果

摘　　要：目的:分析活动义齿和固定义齿修复在牙周病伴牙列缺损患者治疗中的效果。方法:选取2021年6月—2023年5月北京市门头沟区中医医院收治的牙周病伴牙列缺损患者96例作为研究对象,随机分为活动组和固齿组,各48例。两组均在控制牙周病后实施义齿修复,活动组实施活动义齿修复,固齿组实施固定义齿修复。比较两组牙齿功能、牙周情况及口腔健康情况。结果:治疗前,两组口腔清洁程度、咀嚼功能、咬合功能、发音功能评分及总分比较,差异无统计学意义(P>0.05);随访3个月后,两组口腔清洁程度、咀嚼功能、咬合功能、发音功能评分及总分升高,固齿组高于活动组,差异有统计学意义(P<0.05)。治疗前,两组牙龈指数、龈沟出血指数比较,差异无统计学意义(P>0.05);随访3个月后,两组牙龈指数、龈沟出血指数降低,固齿组低于活动组,差异有统计学意义(P<0.05)。治疗前,两组口腔健康影响程度量表(OHIP-14)评分比较,差异无统计学意义(P>0.05);随访3个月后,两组OHIP-14评分升高,固齿组高于活动组,差异有统计学意义(P<0.05)。结论:固定义齿修复在牙周病伴牙列缺损患者中的应用效果优于活动义齿,可提高牙齿功能,维持良好的牙周状态,改善口腔健康水平。Objective:To analyze the effect of removable and fixed denture prostheses in the treatment of patients with periodontal disease with dental defects.Methods:Ninety-six patients with periodontal disease accompanied by dental defects admitted to Bei⁃jing Mentougou District Hospital of Traditional Chinese Medicine from June 2021 to May 2023 were selected as the study sub⁃jects,and they were randomly divided into the removable denture group and fixed denture group,with forty-eight cases in each group.Both groups implemented denture prosthesis after controlling periodontal disease,the removable denture group imple⁃mented active denture prosthesis,and the fixed denture group implemented fixed denture prosthesis.The dental function,peri⁃odontal condition and oral health of the two groups were compared.Results:Before treatment,the difference in oral cleanliness,masticatory function,occlusal function,pronunciation function score and total score between the two groups was not statistically significant(P>0.05);after 3 months of follow-up,oral cleanliness,masticatory function,occlusal function,pronunciation function score and total score of the two groups were elevated,and the fixed denture group was higher than that of the removable denture group,and the difference was statistically significant(P<0.05).Before treatment,the gingival index and gingival sulcus bleeding index of the two groups were compared,and the difference was not statistically significant(P>0.05);after 3 months of follow-up,the gingival index and gingival sulcus bleeding index of the two groups were reduced,and the fixed denture group was lower than the removable denture group,and the difference was statistically significant(P<0.05).Before treatment,the oral health impact scale(OHIP-14)scores of the two groups were compared,and the difference was not statistically significant(P>0.05);after 3 months of follow-up,the OHIP-14 scores of the two groups were elevated,and the fixed denture group was higher than the remov⁃able denture group,and the differe
