慢性牙周炎患者应用牙种植修复治疗的效果研究

摘　　要：目的研究慢性牙周炎患者应用牙种植修复治疗的效果。方法选取80例行牙种植修复治疗的患者,其中40例为牙周健康者(对照组),40例为慢性牙周炎患者(观察组)。两组均实施牙种植修复治疗。比较两组治疗前后牙周指数、基质金属蛋白酶[基质金属蛋白酶-9(MMP-9)、基质金属蛋白酶-8(MMP-8)、基质金属蛋白酶-2(MMP-2)]、咀嚼功能评分;分析观察组牙槽骨吸收情况。结果治疗前,观察组牙周探诊深度、龈沟出血指数、菌斑指数、牙龈乳头指数分别为(1.58±0.20)mm、(0.28±0.13)、(0.50±0.13)、(2.78±0.56),均高于对照组的(1.43±0.15)mm、(0.14±0.11)、(0.40±0.12)、(2.32±0.57)(P<0.05)。治疗后,观察组患者的牙周探诊深度、龈沟出血指数、菌斑指数分别为(1.54±0.23)mm、(0.25±0.17)、(0.47±0.15),与对照组的(1.56±0.21)mm、(0.21±0.16)、(0.49±0.14)相差不明显(P>0.05),观察组牙龈乳头指数(2.05±0.50)较对照组的(0.98±0.42)更高(P<0.05)。治疗前后,观察组MMP-9、MMP-8、MMP-2均显著高于对照组(P<0.05);治疗前后,两组咀嚼功能评分比较相差不明显(P>0.05)。观察组患者治疗1个月牙槽骨吸收(1.29±0.25)mm,治疗3个月牙槽骨吸收(1.52±0.39)mm,治疗5个月牙槽骨吸收(1.69±0.47)mm。结论对慢性牙周炎患者执行牙种植修复法效果明显。Objective To study the effect of dental implant restoration in patients with chronic periodontitis.Methods 80 patients with dental implant restoration were selected,including 40 cases of healthy periodontitis patients(control group)and 40 cases chronic periodontitis patients(observation group).All patients received dental implant restoration.The periodontal index,matrix metalloproteinase[matrix metalloproteinase-9(MMP-9),matrix metalloproteinase-8(MMP-8),matrix metalloproteinase-2(MMP-2)]and masticatory function scores before and after treatment were compared between the two groups;alveolar bone resorption was analyzed in the observation group.Results Before treatment,the periodontal probing depth,sulcus bleeding index,plaque index and gingival papilla in the observation group were(1.58±0.20)mm,(0.28±0.13),(0.50±0.13)and(2.78±0.56),which were higher than(1.43±0.15)mm,(0.14±0.11),(0.40±0.12)and(2.32±0.57)in the control group(P<0.05).After treatment,the periodontal probing depth,sulcus bleeding index,and plaque index of the observation group were(1.54±0.23)mm,(0.25±0.17),and(0.47±0.15),which were not significantly different from(1.56±0.21)mm,(0.21±0.16),and(0.49±0.14)of the control group(P>0.05).The gingival papilla was(2.05±0.50)in the observation group,which was higher than(0.98±0.42)in the control group(P<0.05).Before and after treatment,MMP-9,MMP-8,and MMP-2 in the observation group were significantly higher than those in the control group(P<0.05).Before and after treatment,there was no significant difference in masticatory function score between the two groups(P>0.05).Alveolar bone resorption of patients in the observation group was(1.29±0.25)mm after 1 month of treatment,(1.52±0.39)mm after 3 months of treatment,and(1.69±0.47)mm after 5 months of treatment.Conclusion The implementation of dental implant restoration for patients with chronic periodontitis has a significant effect.
