基于DRG的医院床位工作效率及合理配置研究

摘　　要：目的 以疾病诊断相关分组(DRG)为基础,引入病例组合指数等指标,对医院各临床科室床位资源利用情况进行综合评价,为床位管理及优化提供合理依据。方法 以四川省某地级市三甲综合医院为研究对象,提取2022年医院临床科室各项指标,使用床位指数图、DRG双维度分析法、床位工作效率分别评价科室床位的运行效率、产出效率、床位配置合理性,最后结合3个分析方法进行综合分析。结果 该院41个可供分析的科室中,有2个优势科室、1个过度消耗医疗资源科室、3个发展科室。结论 建议医院针对各类型科室进行床位调整,优化卫生资源分配,加大对床位资源的评价及管理。Objective To introduce the case-mix index and other indicators,comprehensively evaluate the use of bed resource utilization in clinical departments in the hospital,and provide a reasonable basis for bed management and optimization based on disease diagnosis-related grouping(DRG).Methods Using a tertiary comprehensive hospital in a prefecture level city in Sichuan Province as the research object,various indicators of clinical departments in the hospital in 2022 were extracted.Taking the bed index chart,the two-dimension analysis method of DRG,and bed work efficiency to evaluate the operational efficiency,output efficiency,and rationality of bed configuration of the department beds.Finally,a comprehensive analysis is conducted by combining the three analysis methods.Results Among the 41 departments available for analysis in the hospital,there are two advantageous departments,one that excessively consumes medical resources and three development departments.Conclusion It is recommended that hospitals adjust their beds for various types of departments,optimize the allocation of health resources,and strengthen the evaluation and management of bed resources.
