摘  要:目的:分析多学科诊疗(MDT)模式联合基于团队教学(TBL)+基于案例教学(CBL)+基于问题的学习(PBL)的MDT-TBL+CBL+PBL新型教学模式(简称MDT-TCP)在乳腺外科住培教学中的应用效果。方法:选取2023年2月—2024年2月在西南医科大学附属医院乳腺外科接受住院医师规范化培训(简称住培)的100名住院医师,采用随机数字表法分为试验组50名和对照组50名,对照组采用讲授教学法(LBL)教学培养模式,试验组采用MDT-TCP教学培养模式。两组教学时长均为6个学时,培训结束后比较两组出科考试成绩、教学效果满意度、教学模式满意度。结果:试验组的理论考核成绩和病例分析成绩高于对照组,差异有统计学意义(P<0.05);两组的操作考核成绩比较,差异无统计学意义(P>0.05)。试验组对10项教学效果的满意度高于对照组,试验组整体教学模式满意率高于对照组,差异有统计学意义(P<0.05)。结论:MDT-TCP教学模式应用于乳腺外科规范化培训教学中效果较好,可提升临床教学质量和住培医师满意度,提高住培医师的学习、协作、沟通与临床思维等综合能力,值得临床推广与运用。Objective To analyze the application effect of multidisciplinary diagnosis and treatment(MDT)combined with team-based teaching(TBL)+case-based learning(CBL)+problem-based learning(PBL)teaching model MDT-TBL+CBL+PBL(MDT-TCP for short)in the resident training of breast surgery.Methods A total of 100 residents who received standardized resident training in the Breast Surgery Department of the Affiliated Hospital of Southwest Medical University from February 2023 to February 2024 were selected as the research objects,and were divided into the experimental group(n=50)and the control group(n=50)according to the random number table method.The control group adopted the lectured teaching method(LBL)teaching and training mode.The experimental group adopted MDT-TCP teaching and cultivation mode.The teaching time of the two groups was 6 class hours.After the training,the exam scores,teaching effect satisfaction and teaching mode satisfaction of the two groups were compared.Results The theoretical test scores and case analysis scores of experimental group were higher than those of control group,and the difference was statistically significant(P<0.05).There was no significant difference between the two groups(P>0.05).The satisfaction of the experimental group on the 10 teaching effects was higher than that of the control group,and the satisfaction rate of the overall teaching mode of the experimental group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusions The application of MDT-TCP teaching mode in the standardized training and teaching of breast surgery has a good effect,which can improve the quality of clinical teaching and the satisfaction of residential training physicians,and improve the comprehensive abilities of learning,collaboration,communication and clinical thinking of residential training physicians,which is worthy of clinical application.
