关于李东垣阴火论在儿科应用的思考

摘　　要：阴火理论是李杲(号东垣)“内伤学说”的重要组成部分,李杲指出阴火致病主要与脾胃虚弱、元气不充、阴火上乘的病因病机相关,故治当以调补脾胃为本,清泻阴火为标,遣方用药“惟当以辛甘温之剂,补其中而升其阳,甘寒以泻其火则愈矣”。临床上,儿童患病常见脾胃虚损及火热亢盛症状,尤以脾胃自身及肺肝系疾病为多,这与儿童“脾常不足”“肝常有余”的特殊生理病理特点相关,同时契合阴火致病特点,故将阴火理论应用于儿科临床往往可获良效。现从阴火理论出发,探析阴火理论与儿科相关病证发生发展的关联性及治法等,以期为阴火理论在儿科的应用提供一定参考。The theory of Yin Fire(阴火)is an important component of LI Gao’s theory of internal injury.LI Gao pointed out that Tthe pathogenesis of Yin Fire is mainly related to the weakness of the spleen and stomach,insufficient vitality,and superior Yin Fire.Therefore,the treatment should be based on regulating and supplementing the spleen and stomach,accompanied by clearing and purging Yin Fire,and prescribing medication.“Only when using spicy,sweet and warm taste medicines to supplement spleen and increase its Yang(阳),and using sweet and cold taste medicines to clear its fire.”In clinical practice,children often suffer from spleen and stomach deficiency and excessive fire and heat symptoms,especially those related to the spleen and stomach itself,as well as lung and liver diseases.This is related to the special physiological and pathological characteristics of“spleen often insufficiency”and“liver often superabundance”in children.The characteristics are in line with the pathogenic characteristics of Yin Fire.Therefore,applying the theory of Yin Fire to pediatric clinical practice often achieves good results.Starting from the theory of Yin Fire,this article explores the correlation and treatment methods between Yin Fire theory and the occurrence and development of pediatric related diseases,in order to provide reference for the application of the Yin Fire theory in pediatrics.
