摘  要:目的探讨沉默信息调节因子2相关酶1(SIRT1)和高迁移率族蛋白B1(HMGB1)在未破裂颅内动脉瘤患者血清中的表达变化及临床意义。方法选取江油市人民医院2020-01—2021-01住院治疗的80例未破裂颅内动脉瘤患者,另选取同期门诊健康体检者40例为对照组。比较2组临床资料和血清SIRT1、HMGB1水平,采用受试者工作特征(ROC)曲线分析血清SIRT1、HMGB1水平对未破裂颅内动脉瘤的评估价值。结果未破裂颅内动脉瘤患者血清SIRT1水平显著低于对照组,HMGB1水平显著高于对照组(P<0.05)。未破裂颅内动脉瘤患者血清SIRT1、HMGB1表达水平与瘤数目、瘤直径显著相关(P<0.05)。ROC曲线显示,血清SIRT1、HMGB1联合预测未破裂颅内动脉瘤的AUC为0.944,敏感性为96.15%,特异性为87.04%,优于各自单独预测(Z二者联合-SIRT1=2.296,P=0.022;Z二者联合-HMGB1=2.759,P=0.006)。结论未破裂颅内动脉瘤患者血清SIRT1显著低表达,HMGB1显著高表达,两者与瘤数目、瘤直径等临床病理特征显著相关,二者联合检测对未破裂颅内动脉瘤患者具有较好的预测价值。Objective To investigate the expressions and clinical significance of silent information regulator factor 2 related enzyme 1(SIRT1)and high mobility group protein B1(HMGB1)in the serum of patients with unruptured intracranial aneurysms.Methods A total of 80 patients with unruptured intracranial aneurysms hospitalized in Jiangyou People’s Hospital from January 2020 to January 2021 were selected,another 40 outpatients with physical examination at the same time were selected as the control group.The clinical data and serum SIRT1 and HMGB1 levels were compared between the two groups.The receiver operating characteristic(ROC)curve was used to analyze the value of serum SIRT1 and HMGB1 levels in evaluating the occurrence of unruptured intracranial aneurysms.Results The serum SIRT1 level in patients with unruptured intracranial aneurysms was obviously lower than that in the control group,and the HMGB1 level was obviously higher than that in the control group(P<0.05).The expression levels of serum SIRT1 and HMGB1 in patients with unruptured intracranial aneurysms were significantly correlated with the number and diameter of aneurysms(P<0.05).The ROC curve showed that the AUC of serum SIRT1 and HMGB1 for predicting the incidence of unruptured intracranial aneurysms was 0.944,the sensitivity was 96.15%,and the specificity was 87.04%,which was superior to their respective independent prediction(Zcombination-SIRT1=2.296,P=0.022;Zcombination-HMGB1=2.759,P=0.006).Conclusion There is a significant lower expression of SIRT1 and a significant higher expression of HMGB1 in the serum of patients with unruptured intracranial aneurysms.Both of them are significantly correlated with the clinicopathological characteristics such as the number of aneurysms and the diameter of aneurysms.The combined detection of SIRT1 and HMGB1 has a good predictive value for patients with unruptured intracranial aneurysms.
