摘　　要：目的探讨脑脊液细胞因子在中枢神经系统感染患者诊断中的价值。方法收集郑州大学第一附属医院2023-01—2024-04中枢神经系统感染的患者55例为感染组,非中枢神经系统感染者56例为非感染组,比较2组临床资料,脑脊液细胞因子IL-1β、IL-6、IL-8、IL-10、IL-17A、IL-22、TNF-α、IFN-γ水平的差异。结果患者高龄,脑脊液细胞因子IL-1β、IL-6、IL-8、IL-10、IL-17A、IL-22、TNF-α、IFN-γ水平增高,均与发生中枢神经系统感染有相关性,P<0.05;Logistic回归分析提示脑脊液IL-6(OR=1.003,95%CI 1.001~1.005)是中枢神经系统感染的独立危险因素;ROC曲线分析显示脑脊液IL-6的AUC=0.912(95%CI:0.859~0.965,P<0.001),敏感度为96.4%。结论脑脊液细胞因子IL-6是中枢神经系统感染的独立危险因素,可作为中枢神经系统感染诊断的参考指标。Objective To investigate the role of cerebrospinal fluid cytokines in the diagnosis of central nervous system infection.Methods A total of 55 patients with central nervous system infection and 56 patients with non-central nervous system infection from the First Affiliated Hospital of Zhengzhou University from January 2023 to April 2024 were collected as the infected group and the non-infected group.Clinical data of the two groups were compared.Differences in levels of cytokines IL-1β,IL-6,I L-8,IL-10,IL-17A,IL-22,TNF-α,IFN-γin cerebrospinal fluid.Results older patients and increased levels of cerebrospinal fluid cytokines IL-1β,IL-6,IL-8,IL-10,IL-17A,IL-22,TNF-α,IFN-γwere associated with central nervous system infection(P<0.05).Logistic regression analysis suggested that cerebrospinal fluid IL-6(OR=1.003,95%CI:1.001-1.005)was an independent risk factor for central nervous system infection.ROC curve analysis showed that CSF IL-6 AUC=0.912(95%CI:0.859-0.965,P<0.001),sensitivity was 96.4%.Conclusion Cerebrospinal fluid cytokine IL-6 is an independent risk factor for central nervous system infection and can be used as a reference index for the diagnosis of central nervous system infection.
