术后护理干预对库欣病患者精神障碍及生活质量的影响

摘　　要：目的探讨术后护理干预在库欣病患者中的临床应用效果。方法应用随机对照法将41例收入上海交通大学医学院附属瑞金医院神经外科的库欣病患者随机分为实验组和对照组。实验组在常规围手术期护理的基础上给予为期2周共3次的教育内容,对照组患者接受常规的治疗和护理。记录患者的一般人口学资料、术前与术后的焦虑抑郁及生活质量评分。结果实验组干预后的焦虑他评评分(HAMA)、抑郁自评评分[HAD(D)]、抑郁他评评分(HRSD)均低于对照组(P<0.05),实验组干预后生活质量评分与对照组比较基本无异,仅在精力(VT)、情感职能(RE)两个维度分值高于术前(P<0.05)。结论护理教育干预有助于降低库欣病患者出院后至首次入院复查之间的焦虑、抑郁情绪,对患者的短期生活质量略有改善,具有一定的临床适用性。Objective To investigate the clinical effect of intervention in patients with Cushing’s disease(CD)after resolution of hypercortisolism.Methods Forty-one CD patients admitted to Neurosurgery Department of Ruijin Hospital,Shanghai Jiao Tong University School of Medicine were randomly divided into intervention group and control group.The patients in the intervention group were given three times of education for two weeks on the basis of routine perioperative treatment,while the patients in the control group were given routine treatment.Demographic data,preoperative and postoperative anxiety and depression,and quality of life scores were recorded.Results The scores of anxiety and depression in the intervention group were significant lower than those in the control group after intervention(P<0.05),and the scores of quality of life in the intervention group were similar to those in the control group after intervention,but the scores of VT and RE were higher than those before intervention.Conclusion Education intervention after operation can reduce the anxiety and depression of CD patients and improve the short-term quality of life.
