摘  要:目的探索门诊经验沟通与医方示善(goodwill)-医方倾听(listening)-医患交流(talking)-医患合作(coopertion)(GLTC)门诊沟通方式对医生的心理影响,为改善门诊医患沟通提供参考。方法由15名医生在医学人文能力实验室内分别以两类方式与30名患者沟通,全程跟进填写相关情绪量表和问卷,对数据进行Mann-whitney U检验,比较医生不同时间点情绪之间的差异。结果医生应用门诊GLTC沟通方式后,大部分情绪分值分布显著低于门诊个人经验沟通(P<0.05);医生对患者的需要聚焦于患者配合医生行医;专家评价医生应用门诊GLTC沟通技能评分高于门诊经验沟通(P<0.05)。结论门诊个人经验沟通后医生负面情绪波动较大,门诊GLTC沟通后医生负面情绪降低,医生在医患沟通中要注意控制负面情绪,医、患、专家三方均反映门诊GLTC沟通方式效果优良,今后需要结合门诊实际,适当减轻门诊医生工作压力。Objective To explore the impact of the outpatient experience communication and GLTC(Goodwill,Li-stening,Talking,Cooperation)communication in outpatient clinics on doctors'communication psychology,and provide reference for improving outpatient doctor-patient communication.Methods 15 doctors communicated with 30 patients in two ways in the medical humanities ability laboratory,followed up and filled out related emotional scales and questionnaires throughout the process.Mann-whitney test was performed on the data to compare whether there were differences in mood between doctors at different time points.Results After the doctors used the outpatient GLTC communication method,most of the emotional score distribution was significantly lower than the outpatient personal experience communication(P<0.05);the doctor's need for the patient focused on the patient's cooperation with the doctor in the practice;the experts rated higher score for the doctor with the outpatient GLTC communication skills than the communication with outpatient experience(P<0.05).Conclusion After personal experience communication in outpatient clinics,doctors'negative emotions fluctuate greatly.After outpatient GLTC communication,doctors'negative emotions decrease.Doctors should pay attention to controlling anger in doctor-patient communication.Doctors,patients,and experts all reflect that the outpatient GLTC communication method is effective.In the future,it is necessary to appropriately reduce the work pressure of outpatient doctors in light of the actual situation of outpatient clinics.
