摘  要:目的探讨^(99)mTc标记的亚甲基二膦酸盐(^(99)mTc-MDP)全身骨扫描+单光子发射计算机断层扫描(SPECT)/CT在儿童McCune-Albright综合征(MAS)诊断中的应用价值。方法回顾性选择2019年1月至2023年9月在外院疑诊MAS,后在首都医科大学附属北京友谊医院核医学科行骨扫描平面及SPECT/CT断层显像的患儿。所有患儿均行^(99)mTc-MDP全身骨扫描+SPECT/CT,记录平面和断层显像病灶数,并随访记录临床诊断结果及相关临床资料。结果经骨扫描检查及进一步临床评估,7例患儿确诊MAS,9例排除MAS诊断。确诊MAS的7例患儿中,6例全身骨扫描+SPECT/CT显示显像剂浓聚灶,分布于颅骨、肋骨、髋骨、尺骨、股骨、胫骨及腓骨,3例较X线平片显示更多病灶;全身骨扫描联合SPECT/CT局部断层显像可较平面显像显示更多病灶,并且排除体外污染、先天变异及体位因素的影响。9例排除MAS诊断的患儿全身骨扫描+SPECT/CT断层扫描结果均为阴性。结论^(99)mTc-MDP全身骨扫描+SPECT/CT能够为诊断及排除MAS综合征提供重要依据,平面扫描图像存在可疑显像剂浓聚灶时,应进一步联合断层扫描。Objective To evaluate the value of^(99)mTc-methylenediphosphonate(^(99)mTc-MDP)whole body bone scan+single photon-emission computed tomography(SPECT)/CT in the diagnosis of McCune-Albright syndrome(MAS)in children.Methods Children who were suspected of MAS at an external hospital from January 2019 to September 2023 and later underwent bone scan plane and SPECT/CT tomography imaging at the Department of Nuclear Medicine,Beijing Friendship Hospital,Capital Medical University were retrospectively selected.All the children underwent^(99)mTc-MDP whole body bone scan+SPECT/CT.The number of lesions shown by whole body bone scintigraphy and SPECT/CT were recorded,and the clinical diagnosis results and related clinical data were recorded during follow-up.Results After bone scan and further clinical evaluation,MAS was confirmed in 7 cases and excluded in 9 cases.Among the 7 children diagnosed with MAS,6 cases demonstrated foci of intense radionuclide uptake in skull,ribs,hip bone,ulna,femur,tibia and fibula,and 3 cases showed more lesions than X-Ray.Whole body bone scan combined with SPECT/CT could reveal more lesions than plane imaging,and exclude the influence of external contamination,congenital variation and postural factors.In 9 children excluded from,the results of whole body bone scan and SPECT/CT were both negative.Conclusion^(99)mTc-MDP whole body bone scan+SPECT/CT can provide an important basis for the diagnosis and exclusion of MAS.When there is suspicious foci of intense radionuclide uptake in the plane scan image,SPECT/CT scan should be performed.
