临床药师对1例急性脑梗死患者的药学监护

摘　　要：临床中药师全程参与北京中医药大学房山医院1例急性脑梗死患者药物治疗的监护,在此期间,药师通过整理和分析住院患者的药物使用情况,结合患者既往病史、现病史,以及住院期间所完善的所有相关检查,在了解患者的诊疗经过的基础上,临床药师提出患者药物治疗方案的建议.药师的药学监护自患者入院时的首诊问诊到治疗过程中的合理用药安全监护,到出院时对患者进行的用药指导,药师对患者进行全程药学监护.药师参与临床并开展临床药学服务,可以提高该院脑病科患者住院期间的用药合理性,及时与临床医生沟通治疗方案,促进药物的合理应用,进而提高医疗质量.Clinical Chinese pharmacists participated in the monitoring of drug treatment for a patient with acute cerebral infarction in our hospital throughout the entire process.During this period,the pharmacist organized and analyzed the drug use of the hospitalized patient,combined with the patient's past and current medical history,as well as all relevant examinations completed during hospitalization.Based on understanding the patient's diagnosis and treatment process,the clinical pharmacist proposed suggestions for the patient's drug treatment plan.The pharmacist's pharmaceutical supervision covers the entire process from the patient's initial consultation upon admission to the rational and safe use of medication during the treatment process,to the patient's medication guidance upon discharge.The pharmacist provides full pharmaceutical supervision for the patient.Pharmacists participating in clinical practice and providing clinical pharmaceutical services can improve the rationality of medication for patients in the Department of Cerebrology during hospitalization,communicate treatment plans with clinical doctors in a timely manner,promote the rational use of drugs,and thus improve the quality of medical care.
