摘　　要：目的探究经腹膜后入路腹腔镜肾上腺切除术(retro-peritoneal laparoscopic adrenalectomy,RLA)对肾上腺肿瘤患者术后氧化应激水平、肾功能及免疫功能的影响。方法选取2020年3月至2023年3月于我院行肾上腺切除术的肾上腺肿瘤患者81例作为研究对象,根据不同手术方式分为T组和R组。T组40例,给予经腹腔入路肾上腺切除术(transperitoneal laparoscopic adrenalectomy,TLA)。R组41例,予以RLA手术。比较两组患者手术相关指标、氧化应激水平、肾功能、免疫功能、中转开放手术情况及术后并发症情况。结果R组患者的手术时间、术中出血量、术后镇痛药物使用量和胃肠道功能恢复时间明显小于T组(P<0.05)。R组血清超氧化物歧化酶、丙二醛、谷胱甘肽过氧化物酶、血清尿素氮、肌酐、β_(2)-微球蛋白、免疫球蛋白A均明显高于T组(P<0.05)。两组患者的中转开放手术率和术后并发症发生率均无明显差异(P>0.05)。结论RLA应用于肾上腺肿瘤患者可明显降低肾上腺切除手术对患者氧化应激、肾功能及免疫功能的影响。Objective To explore the effects of retro-peritoneal adrenalectomy(RLA)on oxidative stress,renal function and immune function in patients with adrenal tumor after surgery.Methods A total of 81 patients with adrenal tumor who underwent adrenalectomy were selected and divided into T group(40 cases)and R group(41 cases)according to different surgical methods from March 2020 to March 2023.The transperitoneal laparoscopic adrenalectomy(TLA)was performed in group T,and RLA was performed in group R.Surgery-related indexes,oxidative stress,renal function,immune function,conversion to open surgery and postoperative complications were compared.Results The operation time,the amount of intraoperative blood loss,postoperative analgesic drug use and gastrointestinal function recovery time of group R were significantly shorter than those in group T(P<0.05).Serum superoxide dismutase,malondialdehyde,glutathione peroxidase,urea nitrogen,creatinine,β_(2)-microglobulin and immunoglobulin A in group R were significantly higher than those in group T(P<0.05).There were no significant differences in the conversion open surgery rate and postoperative complication rate between the two groups(P>0.05).Conclusion RLA can significantly reduce the effects of adrenal resection on oxidative stress,renal function and immune dynamics in patients with adrenal tumor surgery.
