摘 要:目的 研究适用于基层医务工作者的上消化道内镜(upper gastrointestinal endoscopy, UGIE)操作培训最佳模式。方法 应用我团队研发的便携式内镜及仿真上消化道模型开展UGIE检查教学培训,并与传统直接临床内镜教学模式对比。在10课时、20课时培训后由带教老师指导两组学员分别完成3例人体胃镜操作,记录操作时间、规范化胃镜图像采集张数,并采用上消化道内镜技术综合评估量表(Global Assessment of Gastrointestinal Endoscopic Skills for Upper Endoscopy, GAGES-UE)进行评分,比较两组差异。结果每组各有4名学员完成教学及考核,两组学员经过20课时培训后各项成绩均优于培训10课时。两组学员培训10课时后,便携内镜教学组UGIE操作时间短于传统教学组[(877.00±104.16)s vs(995.04±105.84)s,P<0.001],GAGES-UE通过咽部操作分项评分高于传统教学组[(3.75±1.15)分vs(3.13±1.08)分,P<0.05];10课时培训后,两组学员UGIE操作规范图片采集张数、GAGES-UE总评分及除咽部操作其他分项评分中差异无统计学意义(P>0.05)。培训20课时后,便携内镜教学组通过咽部操作评分以及维持内镜视野清晰评分高于传统教学组[(4.63±0.65)分vs(3.13±1.08)分,P<0.05;(3.50±0.59)分vs(2.75±0.61)分,P<0.05];而在胃镜检查总时间、规范化采集图片张数、GAGES-UE总评分及3个分项评分(循腔进镜熟练度、顺利进入十二指肠降部、胃镜检查质量)方面两组间差异无统计学意义(P>0.05)。结论 UGIE检查新型教学模式在通过咽部关键部位方面优于传统内镜检查教学模式,其前期培训可在医院外进行,减少培训所需患者例数,更适用于基层医务工作者教学培训。Objective To investigate the optimal training model for upper gastrointestinal endoscopy(UGIE)suitable for medical workers in grassroots units.Methods The portable endoscope developed by our team and the simulated upper gastrointestinal model were used for UGIE training,compared with the traditional direct clinical endoscopy teaching mode.After 10 and 20 training hours of training,both groups,under the guidance of supervisory instructors,performed 3 human gastroscopy procedures separately.The operation time,number of standardized gastroscopy images were recorded,and the Global Assessment of Gastrointestinal Endoscopic Skills for Upper Endoscopy(GAGES-UE)were scored.The differences between the two groups were assessed.Results In each group,4 trainees completed the training and assessments.The trainees after 20 training hours in both groups performed better in all aspects than those trained for 10 training hours.After 10 training hours,the portable endoscope teaching group demonstrated shorter total UGIE times[(877.00±104.16)s vs(995.04±105.84)s,P<0.001]and higher GAGES-UE pharynx passage scores[(3.75±1.15)scores vs(3.13±1.08)scores,P<0.05]compared to the traditional teaching group.No significant statistical differences were found between these two groups in terms of standardized image collection counts,GAGES-UE total score and other sub-scores after 10 training hours.After 20 training hours,the portable endoscope teaching group had higher scores in GAGES-UE pharynx passage scores[(4.63±0.65)scores vs(3.13±1.08)scores,P<0.05]and clarity maintenance[(3.50±0.59)scores vs(2.75±0.61)scores,P<0.05].There were no significant differences in the total gastroscopy time,standardized image counts,GAGUES-UE total scores,and three sub-items between these two groups(P>0.05).Conclusion The novel teaching model for UGIE examination is superior to the traditional model in terms of passing through critical parts like pharynx.Meanwhile,the preliminary training for novel teaching model can be conducted outside the hospital,red
