牙周基础治疗联合整形性切除术治疗牙龈瘤的临床研究

摘　　要：目的:探讨整形性切除术应用于牙龈瘤的治疗效果。方法:收集40例牙龈瘤患者临床资料,随机分为对照组与观察组各20例;2组均进行完善的牙周基础治疗,对照组采用传统手术,观察组行牙龈瘤整形性切除;于术前及术后2周、6个月记录2组牙龈瘤部位的角化龈宽度及牙龈退缩情况,观察术后6个月、12个月牙龈瘤复发情况。结果:术后2周、6个月观察组的角化龈宽度均高于对照组(P<0.05),牙龈退缩程度均低于对照组(P<0.05);2组术后6个月、12个月均未见牙龈瘤复发。结论:牙龈瘤患者采用整形性切除法并保留上皮及部分结缔组织,早期获得良好根面覆盖,避免牙龈退缩,获得良好功能及美学效果。Objective:To investigate the effect of plastic resection on epulis.Methods:Clinical data of 40 patients with epulis were collected and randomly divided into control group and observation group,which were 20 cases in each group,respectively.The two groups received complete periodontal basic treatment.The control group received traditional surgery,and the observation group received plastic resection of epulis.The keratinized gingival width and gingival receding were recorded before operation,2 weeks after operation and 6 months after operation.The recurrence of epulis was observed at 6 months and 12 months after operation.Results:The keratinized gingival width of the plastic resection group was significantly higher than that of the traditional surgery group at 2 weeks and 6 months after surgery(P<0.05),and gingival recession was significantly lower than that of the traditional surgery group at 2 weeks and 6 months after surgery(P<0.05).There was no recurrence of epulis in either group at 6 months or 1 year after surgery.Conclusion:For patients with epulis,using plastic resection can preserve the epithelium and part of the connective tissue to obtain good root surface coverage early,avoiding gingival receding,and obtaining good functional and aesthetic results.
