摘  要:目的:通过两样本孟德尔随机化方法探索维生素D与乳腺癌的因果关系。方法:以公开发表汇总水平的全基因组关联分析(GWAS)数据为基础,利用与维生素D强相关的单核苷酸多态性(SNP)作为工具变量,采用逆方差加权、加权中位数等五种分析方法评估因果关联。使用Cochran’s Q检验进行异质性分析,并使用MR-Egger回归进行多效性评估,最后使用“留一法”进行敏感性分析。结果:逆方差加权法显示血清维生素D水平与乳腺癌之间不存在因果关系(P>0.05),其余四种方法也显示均无因果关系,异质性检验显示SNP之间存在一定的异质性(P<0.05)。MR-Egger回归分析结果表明纳入的SNP不存在水平多效性(P>0.05)。“留一法”分析显示没有单个SNP明显影响总体的效应估计。结论:本研究显示血清维生素D水平与乳腺癌之间不存在因果关系,补充维生素D对于降低乳腺癌的发病率可能没有帮助。Objective:To explore the causal relationship between vitamin D and breast cancer using the two-sample Mendelian randomization method.Methods:Based on publicly available summary-level genome-wide association study(GWAS)data,single nucleotide polymorphism(SNP)strongly associated with vitamin D were used as instrumental variables.The causal association was assessed using five different analytical methods,including inverse-variance weighting and weighted median.Heterogeneity analysis was conducted using Cochran’s Q test,and pleiotropy was assessed with MR-Egger regression.Finally,a leave-one-out approach was used for sensitivity analysis.Results:The inverse-variance weighting method showed no causal relationship between serum vitamin D levels and breast cancer(P>0.05),and the other four methods also indicated no causal relationship.The heterogeneity test showed some heterogeneity among SNP(P<0.05).MR-Egger regression analysis indicated no horizontal pleiotropy in the included SNP(P>0.05).The leave-one-out analysis showed that no single SNP significantly affected the overall effect estimate.Conclusion:This study indicates that there is no causal relationship between serum vitamin D levels and breast cancer,suggesting that supplementing with vitamin D may not be helpful in reducing the incidence of breast cancer.
