综合心理干预和技能训练对稳定期精神分裂症患者认知功能的影响

摘　　要：目的:探讨综合心理干预和技能训练对改善稳定期精神分裂症患者认知功能的效果。方法:纳入2020年7月至2022年12月在天津市安定医院住院及门诊的稳定期精神分裂症患者80例随机分为2组。研究组(n=40)在药物治疗基础上施行综合心理干预和技能训练。对照组(n=40)只接受医院提供的一般药物治疗。在基线、干预结束后(第12周)评估症状严重程度和认知功能。结果:治疗12周后,研究组的阳性和阴性症状量表总分、阴性症状量表和一般精神病理量表分均低于对照组(P均<0.05);威斯康星卡片分类测试持续性错误数和非持续性错误数均低于对照组(P均<0.05),Stroop色词测验卡片A、B、C耗时、反映干扰量的耗时数和正确率均低于对照组(P均<0.05),卡片A、B、C正确率均高于对照组(P均<0.05),数字符号替换测验正确反应数高于对照组(P<0.05),连线测验-A、连线测验-B反应时间均低于对照组(P均<0.05)。结论:综合心理干预和技能训练有助于减少稳定期精神分裂症患者的阴性症状、改善认知功能,是一种有效且成本低、效益高的认知功能障碍治疗方法。Objective:To investigate the effect of integrative psychosocial and skills training on improving cognitive function in patients with stable schizophrenia.Method:80 patients with stable schizophrenia in Tianjin Anding Hospital from July 2020 to December 2022 were included and randomly divided into 2 groups.The trial group(n=40)received integrative psychosocial and skills training.The control group(n=40)received only the general medication provided by the hospital.Symptom severity and cognitive function were assessed at baseline,at the end of the intervention(week 12).Results:After 12 weeks of intervention,The total score,negative scale,and general psychopathology scale scores of positive and negative syndrome scale in the trial group were lower than those in the control group(all P<0.05);The numbers of Rpe and nRpe in Wisconsin card sorting test were lower than that in the control group(all P<0.05);The time consumption,Stroop interference effect time consumption,and accuracy of cards A,B,and C in Stroop color word test were lower than those in the control group(all P<0.05),while the accuracy of cards A,B,and C were higher than that in the control group(all P<0.05);The number of correct digit symbol substitution test responses was higher than the control group(P<0.05);The reaction times of trail making test A and B were lower than that of the control group(all P<0.05).Conclusion:Integrative psychosocial and skills training can help reduce negative symptoms and improve cognitive function in patients with stable schizophrenia,and is an effective and cost-effective treatment for cognitive dysfunction.
