Ⅱ、Ⅲ期乳腺癌患者术后三维适形放疗所致放射性肺损伤的危险因素探究

摘　　要：目的:分析Ⅱ、Ⅲ期乳腺癌术后三维适形放疗所致放射性肺损伤危险因素。方法:选取2020年1月至2022年1月期间于本院诊治的138例Ⅱ、Ⅲ期乳腺癌术后三维适形放疗患者作为研究对象。根据患者放疗后是否发生放射性肺损伤(Radiation-induced lung injury,RILI),将患者分为RILI组(n=30)和非RILI组(n=108)。收集所有患者的临床资料,采用Logistic回归分析患者术后三维适形放疗发生RILI的危险因素。结果:两组的年龄、临床分期、患者侧肺Dmeam、V_(5)、V_(10)、V_(15)、V_(20)、V_(25)等方面比较,差异具有统计学意义(P>0.05)。Logistic回归分析结果显示,存在慢性阻塞性肺炎、V_(20)是Ⅱ~Ⅲ期乳腺癌患者术后三维适形放疗所致RILI的危险因素。结论:Ⅱ~Ⅲ期乳腺癌患者术后三维适形放疗所致RILI的危险因素为慢性阻塞性肺炎、V20。Objective:To analyze the risk factors of lung injury induced by three-dimensional conformal radiotherapy in stageⅡ-Ⅲbreast cancer.Methods:A total of 138 patients with stageⅡ-Ⅲbreast cancer who underwent three-dimensional conformal radiotherapy in the hospital were enrolled as the research objects between January 2020 and January 2022.Patients were categorized into two groups based on the presence or absence of radiation-induced lung injury(RILI)following radiotherapy:RILI group(n=30)and non-RILI group(n=108).Clinical data for all patients were collected,and Logistic regression analysis was conducted to identify the risk factors associated with RILI after three-dimensional conformal radiotherapy.Results:Significant differences were observed in age,clinical staging,Dmean,V_(5),V_(10),V_(15),V_(20),and V_(25)of the lateral lung between the two groups(P>0.05).Logistic regression analysis indicated that chronic obstructive pneumonia and V_(20)were identified as risk factors for RILI induced by three-dimensional conformal radiotherapy in patients with stageⅡ-Ⅲbreast cancer.Conclusion:Chronic obstructive pneumonia and V_(20)are established as risk factors for RILI induced by three-dimensional conformal radiotherapy in patients with stageⅡ-Ⅲbreast cancer.
