硬膜外麻醉下靶控输注丙泊酚复合瑞芬太尼对胃癌根治术疼痛评分及T淋巴细胞亚群的影响

摘　　要：目的探讨对于胃癌根治术患者使用硬膜外麻醉下靶控输注丙泊酚复合瑞芬太尼的麻醉效果以及对T淋巴细胞亚群的影响。方法选取我院2021年04月-2023年09月行胃癌根治术的患者84例,分为对照组、观察组。对照组行气管插管靶控注射丙泊酚复合瑞芬太尼全麻,观察组使用硬膜外麻醉下靶控输注丙泊酚复合瑞芬太尼。对比麻醉恢复、镇静评分、镇痛评分以及T淋巴细胞亚群。结果在手术之后,两组患者的自主恢复时间、苏醒时间无显著性差异(P>0.05),观察组的拔管时间、诉求镇痛时间均明显低于对照组,定向力恢复时间高于对照组,有显著性(P<0.05);麻醉后,观察组HR、BIS、MAP水平均明显比对照组要低,有显著性(P<0.05);手术后1 h、4 h,观察组的VAS评分明显比对照组低,Ramsay评分比对照组高,有显著性(P<0.05);手术后1 d,观察组CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)水平明显比对照组高,有显著性(P<0.05)。结论胃癌根治术患者使用硬膜外麻醉下靶控输注丙泊酚复合瑞芬太尼的麻醉效果较好,值得推广。Objective Consider anesthesia called propofol under epidural anesthesia in patients with stomach cancer,as well as exposure to lymphocytosis t.consider anesthesia called propofol under epidural anesthesia in patients with stomach cancer,as well as exposure to T lymphocytosis.Methods To select 84 patients,from April 2021 to September 2023,who in our hospital are exposed to.fundamental therapy for stomach cancer,randomly divided into control and observation groups.The control group intuits propofol in propofol,and the group USES epidural anesthesia to inject propofol into the rifentanil complex.Compare it with the recovery of a patient under anesthesia,analysis of sedatives,analgesics and lymphocytic group T.Results After the surgery,the two groups of patients did not have significant differences in the time of self-recovery,awakening time(P>0.05),and the observation group had much less time for recovery and treatment of analgeic diseases than the control group,which was significant(P<0.05);After anaesthesia,hr,bis and map levels in observer group are lower than control group and are of significant importance(P<0.05);After operation 1h,4h,observation groups have significantly lower vas scores than the control group,as well as significantly higher ramsay scores,which is essential(P<0.05);After surgery 1 d,CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)in patients with observation group was higher than control group(P<0.05).Conclusions Patients suffering from stomach cancer use epidural epidural anesthesia to inject propofol into the rifentanyl complex,which is more effective and deserves to be raised.
