基于牙周微生态评估显微根管术治疗牙髓牙周并发畸形根面沟的疗效

摘　　要：目的:基于牙周微生态评估显微根管术治疗牙髓牙周并发畸形根面沟的疗效。方法:以2020年10月至2023年10月于本院接受治疗的牙髓牙周并发畸形根面沟患者118例为研究对象,采用简单随机法分为两组。对照组59例予以牙周组织再生术治疗,观察组59例予以显微根管术联合牙周组织再生术治疗。对比两组手术前后的牙周菌群分布情况、牙龈指数(GI)、龈沟出血指数(SBI)、菌斑指数(PLI)、牙齿松动度(TM),评价两组的疗效及安全性。结果:术后1周,观察组的牙周中口腔链球菌、牙龈卟啉单胞菌、福赛类杆菌的检出率低于对照组(P<0.05);术后1周,观察组的GI、SBI、PLI、TM较对照组均降低(P<0.05);观察组的临床疗效优于对照组,两组均未发生明显的并发症。结论:显微根管治疗牙髓牙周并发畸形根面沟的疗效显著,可有效改善牙周菌群及微生态环境,改善牙齿松动度。Objective:To evaluate the effectiveness of microscopic root canal treatment based on periodontal microbial ecology assessment in the treatment of radicular groove-associated pulpal-periodontal lesions.Methods:A total of 118 patients diagnosed with radicular groove-associated pulpal-periodontal lesions from October 2020 to October 2023 were randomly divided into two groups:the control group(n=59)treated with periodontal tissue regeneration surgery,and the observation group(n=59)treated with microscopic root canal treatment combined with periodontal tissue regeneration surgery.The distribution of periodontal microbiota,gingival index(GI),sulcus bleeding index(SBI),plaque index(PLI),tooth mobility(TM),as well as treatment efficacy and safety were compared between the two groups before and after surgery.Results:At 1 week postoperatively,the observation group showed lower detection rates of Streptococcus mutans,Porphyromonas gingivalis,and Fusobacterium nucleatum in periodontal pockets compared to the control group(P<0.05).The observation group also exhibited lower GI,SBI,PLI,and TM compared to the control group at 1 week postoperatively(P<0.05).The clinical efficacy in the observation group was superior to that in the control group,and both groups did not experience significant complications.Conclusion:Microscopic root canal treatment for radicular groove-associated pulpal-periodontal lesions demonstrates significant effectiveness,effectively improves periodontal microbiota and microbial ecology,and reduces tooth mobility.
