漏诊宫颈鳞癌卵巢转移1例并文献复习

摘　　要：宫颈癌是妇科常见的恶性肿瘤之一,其发生卵巢转移率较低,临床应予以重视,提高诊断效率。本文回顾分析重庆市第五人民医院收治的1例漏诊宫颈鳞癌并发卵巢转移患者的诊治过程,患者11+个月前外院行宫颈活检提示宫颈高级别鳞状上皮内病变,未规范随访及治疗,入本院前1个月体检发现异常后行宫颈环形电切术,病理检查结果提示宫颈鳞状上皮高级别内瘤变伴累及腺体,进一步行手术治疗,最终病理提示宫颈鳞癌,累及宫体、输卵管和卵巢,术后行补充放射治疗。通过该病例和相关文献复习,可使妇科医师对宫颈鳞癌卵巢转移的高危因素、转移方式、诊断和治疗有一定了解。Cervical cancer is one of the most common malignant tumors in gynecology,the incidence of ovarian metastasis of cervical cancer is low,which should be paid more attention to clinic and improve the diagnostic efficiency.In this paper,the diagnosis and treatment process of a patient with missed cervical squamous cell carcinoma complicated with ovarian metastasis admitted to The Fifth People's Hospital of Chongqing was reviewed and analyzed,the cervical biopsy performed 11+months ago in another hospital indicated high-grade squamous intraepithelial lesions of the cervix,and the follow-up and treatment were not standardized,cervical circular electrotomy was performed after physical examination revealed abnormalities at 1 month before admission to our hospital,pathological examination showed that cervical squamous epithelial high-grade intraepithelial neoplasia with involvement of glands,and further surgical treatment was performed,the final pathology revealed cervical squamous carcinoma involving uterine body,fallopian tubes and ovaries,and supplementary radiotherapy was performed after surgery.Through the review of this case and related literature,gynecologists can have a certain understanding of the risk factors,metastasis mode,diagnosis and treatment of ovarian metastasis of cervical squamous cell carcinoma.
